EMPLOYEE WAS HELPING LOAD TRUCK OF BULK ATTAPULGITE A. THE SCREW CONVEYOR BEING USED FOR LOADING BOGGED DOWN. EMPLOYEE WAS TRYING TO FIND OUT THE CAUSE. HE STUCK HIS HAND BETWEEN THE UNLOADING CONVEYOR SPOUT OF TANK AND SCREW CONVEYOR. THE CONVEYOR STARTED AND CAUGHT EMPLOYEES FINGERS CUTTING OFF ENDS OF INDEX AND MIDDLE FINGERS - RIGHT HAND
