While crawling out from a screen deck, employee dropped one foot to scaffold board. The board broke and employee fell, injuring right middle finger, and slid down chute to the conveyor belt.
Employee had a personal injury due to a fall at home on July 4th. After working the shift on 08/03/2009, he went to his personal doctor for a check-up but while there claimed his back and left shoulder hurt him and it was from 'working too fast in the loader'.

Maintenance employee was helping to install a differential in a loader. He was using an air jack which he placed on top of a RR tie for easy access. Employee's hand was resting on the ground next to tie when jack fell off and impacted hand. Employee denied medical treatment until 9/2/08 when we sent him to the hospital for x-rays and discovered his hand was broken.

Employee was driving haul truck when he felt something crawling (insect) under his left pant leg along his calf. The employee swatted his leg and it went away. The next day he had some swelling and complained of some pain. The initial visit the doctor observed his injury and told him his options. The follow-up visit the doctor provided him with some antibiotics (prescription).

The employee then went to the doctor again (12/4/06) and received physical therapy treatment to resolve issue.

There were no guards in place to protect the employee. The guards were previously removed some time in the past and were not replaced. Employee heard a noise coming from the fan area and stuck his hand up in the area as a natural reaction. The fan blade cut two of his fingers.
