EE WAS STEPPING DOWN FROM THE CONVEYOR TO THE CATWALK WHERE HE HAD LAID A SHOVEL. HE STEPPED ON THE SHOVEL. INJURING LEFT ANKLE.
EMPLOYEE WAS OPERATING A COMPACTOR WHICH SLID DOWN AND CAUSED HIM TO PULL BACK ON IT, HE THEN FELT A PULL IN THE BACK OF HIS NECK.
EE WAS GUIDING THE REBAR FOR CORRUGATED PIPE. THE RIGGING HOLDING THE REBAR BROKE AT THE WIRE TIES CAUSING THE BANDS TO CUT AND PINCH HIS HAND AND FINGER.
