Employee was trying to remove a bearing from the bearing housing. EE was using a sledge hammer to knock the bearing loose. When EE struck the head of the sledge hammer against the bearing a small piece of metal flew off the bearing and punctured left leg. Employee received one stitch.
Employee was shoveling around the pea gravel belt. EE felt something sting or bite EE's chest. Employee went to the doctor because of the swelling of the bite. Doctor had to lance the insect wound.

Employee slipped and fell while trying to enter a dozer. Employee fell 2.5 feet and landed on his right shoulder. The employee felt fine after the incident. He continued to work for eight days without any issues. On 1/12/2016 employee went to his doctor because of dizziness and a headache. The doctor diagnosed the employee with a slight concussion that was caused by the fall.

While checking front windshield wiper, the wiper became stuck, emplyee got out and found a 1.5 inch diameter rock stuck at the base of the wiper, he grabbed the rock with his left index finger and thumb and pulled back on it, the rock and the wiper started to move back to the start position, catching the left thumb between the wiper arm assembly, caused a fracture on left thumb.

STRAIN TO RIGHT ELBOW WHILE TRYING TO START AIR COMPRESSOR. INJURY OCCURRED 5/22/00 BUT HE DID NOT WAT TO SEE DOCTOR UNTIL NOW (JUNE 12). DOCTOR DIAGNOSED BURSITIS TO ELBOW AND ORDERED 2 WEEKS REST BEFORE RETURNING TO REGULAR DUTY.
