Debris such as drink bottles, where left on the front steps going into the control room.

Injured was holding a coil of disconnected oxygen/acetylene hoses while a fellow miner cut the plastic tie straps off the coil with a knife. When the knife went through the plastic, it deflected the knife upwards into the injured hand. The 3cm laceration needed 6 stitches to close.
