Rain and wet conditions led to the accident. Equipment and equipment ladder was in good working order, operator had on EE's PPE, and three points of contact was being implemented. Operator suffered a closed displaced fracture of shaft of fourth metacarpal bone of left hand. Operator's foot slipped on ladder rung causing EE to slip and hand got caught on the ladder.

Struck by falling object
Quarry personnel were performing a steel change, concave for cone crusher. New concave placed on blocks of wood at the crusher before using the crane to place in crusher. A block of wood got knocked out and a miner's foot was underneath when it tipped and trapped right foot. The weight of the concave broke big toe.

Customer driving a 4 axle dump truck could not get tailgate closed after being loaded with gravel. Driver used hand instead of a hand-held tool to free a rock that was preventing latch from closing. Upon removal of the rock the latch closed on the driver's hand removing skin from the area between pointer finger and thumb.
