Rain and wet conditions led to the accident. Equipment and equipment ladder was in good working order, operator had on EE's PPE, and three points of contact was being implemented. Operator suffered a closed displaced fracture of shaft of fourth metacarpal bone of left hand. Operator's foot slipped on ladder rung causing EE to slip and hand got caught on the ladder.

Quarry personnel were performing a steel change, concave for cone crusher. New concave placed on blocks of wood at the crusher before using the crane to place in crusher. A block of wood got knocked out and a miner's foot was underneath when it tipped and trapped right foot. The weight of the concave broke big toe.

Customer driving a 4 axle dump truck could not get tailgate closed after being loaded with gravel. Driver used hand instead of a hand-held tool to free a rock that was preventing latch from closing. Upon removal of the rock the latch closed on the driver's hand removing skin from the area between pointer finger and thumb.

HE WAS DRIVING A WEDGE BEHIND A SEGMENT ON A 42-65 CRUSHER WHEN IT SUDDENLY POPPED OFF DROPPING 3FT ONTO HIS RT FOOT HITTING IT JUST ABOVE THE STEEL TOE ON HIS BOOT
EMPLOYEE WAS TAKING CRUSHED GRAVEL FROM BINS. HE FORGOT TO PUT THE BODY OF THE TRUCK DOWN BEFORE GOING UNDER THE BIN. HE DAMAGE THE TRUCK, BINS AND CUT HIS NOSE ON THE SUN VISOR IN THE TRUCK. HE HAD HIS SEATBELT ON. AND THE CUT WAS CLEANED AND BANDAID APPLIED BY THE DOCTOR.
CAT TRUCK WAS TRAVELING AT A HIGH RATE OF SPEED WHEN IT CONTACTED THE ROCK BERM ON THE QUARRY ROAD. IT WAS FORCED UP ONTO THE ROCK BERM THEN TIPPED OVER ONTO IT'S SIDE. DRIVER WAS WEARING HI S SEATBEWLT AND RECIEVED A BRUISE ON HIS FOREHEAD. HE WAS ADVISED TO TAKE THE REST OF THE DAY OFF AS HE WAS SHOOK UP.
EMPLOYEE WAS CUTTING DOWN PLASTIC CURTAINS UNDER PLANT BINS HIS KNIFE SLIPPED AND HE CUT HIS ARM

WELDING TRUCK W5S BACKED INTO FENCE AND PINCHED EMPLOYEE WAS SUPECTED AND THE DRIVER OF THE VEHICLE WAS TERMINATED. THE INJURED EMPLOYEE ORIGINALLY CAME BACK TO WORK BUT HAS SINCE LOST TIME.
