Employees were changing a screen at VS-2. After LOTO, the screen was unbolted and secured and as employee was positioning the screen for removal, the screen shifted and pinned employees finger between the screen and the lip / edge of the chute.

Individual was greasing the lift arms on a 988F Caterpillar loader. He slipped and fell to the ground about 18 to 24 inches. Reported that he twisted his knee. He did refuse treatment and or to go to the doctor as noted on company sign off accident report.

Unit being repaired by dealer mechanic-unit was over heating hydraulic fluid. Dealer mechanic being instructed by manufacturer tech service to bypass thermal valve, sending fluid directly to cooler system-hydraulic line ruptured spraying fluid-flash fire burning operator (our operator) on back and arms (2nd degree). Dealer mechanic received minor burns on hand and forehead.
