WHILE EXAMINING THE BLEREDER (5WEST) A ROCK FALL WAS DISCOVERED IN #5 ENTRY OF 1ST WEST AT 5.5#9810 FALL WAS IN A 3WAY INTERSECTION IT WAS APP 3FT LONG & 20FT ACROSS THE ENTRY & APP 6FT HIG H NO MEN OR EQUIP AFFECTED NO VENTILATION AFFECTEDIT WILL BE DANGERED OFF PASSAGE OF MEN IS NOT AFFECTED IT CAN BE BY PASSED
HE WAS HAULING DIRT TO COVER REFUSE ON MOUNTAINS. SECOND LOAD HE GOT OUT TO CHECK BERM. WHILE MOUNTING THE OFF ROAD TRUCK HIS LEFT FOOT SLIPPED THROUGH THE LADDER ON SECOND STEP. THIS CAUSED H IM TO LOSE HIS BALANCE AND FALL BACKWARDS TO THEGROUND. CHIPPED BONE IN LEFT FOOT.
HE WAS HELPING TO DROP R.R. CARS INTO THE UPPER CAR STORAGE AREA. HE WAS IN PROGRESS OF LOOSENING HAND BRAKE ON A CAR WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
HE WAS ROCKDUSTING TRACKWAY IN LOW TOP AREA 4 INCH CLEARANCE WHEN THE DUST HOSE FELL DOWN INTO THE DECK WITH HIM. IN HIS EFFORTS TO SET AWAY HE BUMPED HIS BACK ON THE TRAM FRAME. HE WAS OFF ON VACATION AND DUE TO MINE BEING IDLE NOT SCHEDO TO WORK UNTIL 8-11-93. AT THIS TIME IT WAS DISCOVERED HE HAD BEEN TO A DR.
A ROOF FALL OCCURRED IN 5 EAST BLEEDERS 5 DEV. #4 ENTRY APP. 60' INBY S.S.#4201. FALL WAS ACROSS THE ENTRY (20') AND 20' LONG. IT WAS APP. 4' TO 5' HIGH AREA HAS BEEN CRIBBED AND DANAERED OFF. NO PEOPLE INVOLVED NO EQUIP. INVOLVED NO VENTILATION AFFATED.
LONGWALL RECOVERY. THEY WERE BUILDING A CRIB AT JACK #21 WHEN DRAW ROCK FELL 1 1/2" X 40" X 47" ON CRIB. CORNER OF ROCK HIT HIM ON NECK AND LEFT SHOULDER. HE WENT OT DR ON 24 & 25 WORKED WEEK OF 28 TO JULY 2. WENT BACK TO DR JULY 6 AND BEGAN MISSING WORK.
HE WAS CROSSING BETWEEN TWO PARKED SUPPLY CARS. HIS LEFT FOOT WAS A COUPLING. WHEN HE CAME DOWN ON THE RIGHT FOOT HE HEARD A LOUD POP AND FELT A SHARP PAIN IN HIS RIGHT ANKL.E
EMPL WAS WORKING ON A BEAM PUMP. EMPL WAS TIGHTENING BOLTS WITH A RACHET. THE RACHET SLIPPED, EMPL FELL FORWARD HITTING THE PUMP WITHHIS FACE. THERE WAS A LACERATION & BRUISE TO THE LEFT EYELID. EMPL WAS AT THE DOCTOR 1 DAY & MINE WAS IDLE 3/1/93 & 3/2/93.

SHEARER WAS COMING TO HEAD WHEN THE RIPPERS CAUGHT JACK HEADER PIECES OF METAL WERE THROWN OUT STRIKING HIM ON THE KNUCKLE OF THE LEFT HAND HE WAS WEARING GLOVES AT THE TIME.
HE WAS WALKING OFF FACE TOWARDS DINNERHOLE WHEN HE SAYS HIS HEAD STRUCK A JACK HEADER HEADER WAS DOWN APP 4-5 INCH DUE TO UNEVEN TOP. HE FELL AND WAS HELPED UP, HE COMPLAINED OF NECK STIFFNESS AND SORENESS.
HE TOLD ME HE GOT DUST IN HIS EYE (LEFT) ON 12-14-92 WHILE TRAVELING WITH INSPECTOR (BACKSIDE OF 4 SOUTH BELT). ON 2-17-92 HIS EYE WAS SWOLLEN AND TEARING. HE FILED ACCIDENT REPORT AND WENT T O DR 18TH. HE HAD PREVIOUSLY SIGNED FOR DAYS. MINE WAS IDLE WEEK OF 12-21-92 VACATION.
HE WAS CUTTING A NOTCH IN TIMBER ALONG BELT TO FACILITATE INSTALATION OF A TOP BELT ROLLER HE WAS TRYING TO KNOCK THE NOTCH OUT WITH AN AXE WHEN THE 6IN AIRLINE RUPTURED THE FORCE KNOCKED HIM DOWN CAUSING INJURY HE HAD MULTIPLE BRUISES LACERATIONS & ABRASIONS
A PIECE OF EICKO-TRAC SLID OFF SHEARER TOP HITTING HIM ON WRIST CAUSING A BRUISE TO THE BASE OF THE RIGHT THUMB.
HE SAID HE WAS CHANGING RETURN EMULSION LINE WHEN SOME EMULSION CAME OUT OF THE END SUDDENLY STARTLED HE JUMPED BACKWARES & IN THE PROCESS HE SAYS HE PULLED A MUSCLE PECTORAL ON LEFT SIDE OF H IS CHEST
WHILE IN THE PROCESS OF RECOVERING RAILS HE USED AN AXE AND WRENCH REPETITIVLY. DURING SHIFT HE SAYS HIE RIGHT WRIST STARTED BOTHERING HIM THE DONDITION WORSENED AND HE REPORTED IT TO HIS SUPE RVISOR ON 11-25-92. HE WENT TO DOCTOR AND STARTED MISSING WORK ON 12-1-92.
THE SHEARER WAS MAKING A COAL RUN FROM TAIL TO HEAD FOLLOWING 1ST IGNITION WHEN ANOTHER IGNITION OCCURRED AT JACK #72. THEY WERE NOT CUTTING ROOF. THE RIPPERS HAD STRAYED BARELY MAKING CONTACT . THE FLAME WAS 2' X 2' AND LASTED FOR ONE MIN. IT WAS PUT OUT WITH WASH HOSE.
THE SHEARER WAS MAKING COAL RUN FROM HEAD TO TAIL. AT JACK #77 THE TAIL DRUM ENCOUNTERED SOME SANDSTONE IN AND AROUND A LARGE KETTLE BOTTOM. THE ARCS IGNITED METHANE FEEDERS HIDDEN IN THE COAL . A BALL OF FLAME CAME OUT FROM THE RIPPERS TO MIDWAY OF THE FACELINE THEN SETTLED DOWN BURNING ALONG THE FEEDER LINES. IT WAS PUT OUT WITH WATER HOSE ON THE SHEARER AFTER BURNING FOR APP 5 M
HE WAS HELPING ROTATE #58 SIEVE SCREEN WHEN HE CAUGHT HIS FINGER BETWEEN SCREEN FRAME AND CROSS BRACE. HE WAS WEARING GLOVES.
THEY WERE HANGING MONO-RAIL.HE SAID WHEN THEY RAISED UP A SECTION HE FELT SOMETHING PULL IN THE MIDDLE OF HIS BACK AND LEFT SIDE.SAW DOCTOR AND BEGAN MISSING WORK ON 11-02-92.
HE WAS CHECKING FITTING IN PREPARATION FOR HOSING UP JACK. THE FITTING FRACTURED LETTING HIGH PRESSURE EMULSION HIT HIM IN FACE AND LEFT CHEST. HEAD INJURIES AND FRACTURED RIBS AND INTERNAL DA MAGE.

THEY WERE TURNING COWLS GETTING READY TO MAKE ANOTHER RUN TO TAIL THE HEAD SIDE RIPPERS CAME IN CONTACT WITH SA 54236NE ROOF AN IGNITION OCCURRED WITH FLAMES COMING FROM THE RIPPERS TOWARDS THE GOB FLAMES SELF EXTINGUISHED.

HE DESCRIBED AN ORANGE GLOW TRAVELIN 70196UND AND AWAY FROM THE HEAD RIPPER RING APPROX. 12". HE SHOUTED, THE RIPPERS STOPPED, THE GLOW DISAPPEARED. THE INCIDENT OCCURRED ON 2 DEV EAST OFF 5 SOUTH.

THE INJURY OCCURRED AS A RESULT OF A PRESSURE BUILT INSIDE THE HEAD RANGING ARM OF THE JOY 4LS SHEARER.HOT OIL BLEW FROM THE MOTOR WHEN SNAP RING WAS REMOVED TO CHANGE A BROKEN TORQUE SHAFT.IN JURED WAS BURNT ON HIS FACE AND FOREARMS.SEVERITY OF INJURIES UNKNOWN AT THIS TIME.
EE WAS INSIDE A BIN USING A WELDING MACHINE TO REMOVE AND REPLACE METAL TO THE LINING OF THE BIN. ANOTHER EE WAS ALSO INSIDE BIN WITH A TORCH DOING METAL WORK.
HE WAS GETTING HIS TOOLS OUT OF WALK IN LOCKER. WHEN HE EXITED HE HIT HIS LEFT KNEE ON SPEED REDUCER STORED IN THE AREA. KNEE WAS SWOLLEN ON 7-7-92 HE WENT TO DR AND BEGAN MISSING WORK BECAUSE OF INJURY.
HE WAS RE-ALIGNING BELT IN THE SUPER TAKE-UP USING PIECE OF DRILL STEEL TO HIT THE BASE OF THE BOTTOM ROLLERS. THE BAR GLANCED LETTING HIS FINGER GET PINCHED AGAINST A BELT STAND.
EMPL INSIDE OLD CHUTE MAKING MEASURMENTS TO MODIFY NEW CHUTE.ON EXITING CHUTE,LEG CAME IN CONTACT WITH A STRAY PIECE OF METAL OF CHUTE LINER.THIS CAUSED A LACERATION REQUIRING STITCHES.
HE WAS WALKING BACK FROM DRAGGING TWO JOINTS 3 INCH DISCHARGE PIPE PLASTIC WHEN HE TUMBLED ON A LARGE ROCK HEAVING BOTTOM AND FELL WHEN HE FELL HIS RIGHT KNEE STRUCK THE ROCK. HE HAD WALKED AP P 2-3 BREAKS WHEN FELL.
ATTEMPTING TO MOUNT A SLOW MOVING R R CAR TO SET BRAKES FOOT SLIPPED CAUSING HIM TO FALL R FOOT WAS PINNED BY WHEEL WHICH SEVERED PART OF FOOT, DRUG HIM APP 20 YARDS
THE SHEARER WAS AT JACK #130 MOVING TOWARDS THE HEAD. THE HEAD RIPPERS INADVERTENTLY HIT SANDSTONE ROOF WHICH RESULTED IN SPARKS IGNITING METHANE AT JACK 133. THE METHANE WAS IN A NARROW SPACE 2" WHERE LARGE LUMP OF COAL HAD SLID DOWN FROM THE ROOF.
THE SHEARER WAS MAKING A COAL RUN FROM TAIL TO THE HEAD. AT JACK #'S 45-46 THE OPERATOR WAS CUTTING TOP TO GAIN HEIGHT FOR CHANGING A HOSE ON JACK #45. WHILE CUTTING TOP AN IGNITION OCCURRED W HICH WAS SELFEXTINGUISHED IMMEDIATELY. THERE WERE NO INJURIES OR EQUIPMENT DAMAGE. INCIDENT OCCURRED ON 3 DEV. EAST OFF 5 SOUTH.
DISCHARGE DOOR ON REFUSE BIN WOULD NOT OPEN. HE HOOKED WIRE ROPE ONTO THE DOOR AND A TOOTH OF END LOADER PUTTING TENSION ON DOOR. HE GOT ON CAB AND WAS HITTING DOOR WITH HAMMER WHEN ROPE CAM E OFF HITTING HIM IN MOUTH. ONE TOOTH BROKEN AND SEVERAL LOOSEN.

THEY WERE LOADING 50 LB. BAGS OF CEMENT ONTO PORTAL BUS. HE DECIDED TO MOVE TWO BAGS TO A LOWER LEVEL. WHEN HE RAISED UP HE FELT A BURNING SENSATION IN HIS LOWER BACK.
HE SAYS HE WAS LIFTING ON BELT STRUCTURE WHEN HE FELT PAIN IN HIS LOWER BACK. HE HAD TAKEN DAYS ON DOMESTIC BACK INJURY 1 WEEK PRIOR TO THIS.
INJURED AND TWO OTHERS WERE MOVING 3" SANDPIPER PUMP ON A TWO WHEEL DOLLY. HE SAID HE WAS PULLING ON DOLLY WHEN WHEEL CAUGHT CASUING A SUDDEN JERK HE SAID HE FELT SUDDEN PAIN IN HIS LOWER BAC K.
A SMALL PIECE OF DRAW ROCK FELL BETWEEN RIB BOLT AND RIB HITTING HIM ON THE HEAD AND SHOULDERS ABRASIONS ON THE RIGHT ARM.
THEY WERE TAKING CRIB BLOCKS OFF SUPPLY CAR. HE WAS PULLING ON A METAL STRAP TO GET IT OUT OF THE WAY. WHEN HE PULLED THE STRAP FLIPPED OUT AND HIT HIM ON THE RIGHT FOREARM.
THEY WERE PREPARING TO TAKE CENTER PINS OUT OF A COAL HAULER. THE COAL HAULER HAD BEEN JACKED UP BY THE 1ST SHIFT WITH A 10 TON RAILROAD JACK. HE STARTED TO LET THE JACK DOWN AND THE JACK BAR CAME OUT AND HIT HIM ON HIS CHIN.
HE WAS PUTTING WRECKED SUPPLY CAR ON TRACK, HAD IT JACKED UP APP. 8" HE REACHED TO GET A CRIB BLOCK UNDER THE CAR AT THE SAME TIME THE JACK CREELED OUT LETTING THE CAR FALL ON HIS LEFT THUMB.
ELECTRICIAN CLEANING TORCH TIPS BY RUBBING THE LIT TORCHES AGAINST A PIECE OF METAL.SPARKS IGNITED A SMALL FEEDER IN MINE FLOOR 2' FROM WHERE HE WAS STANDING.FLAME WAS 2-3"HIGH AND COLOR WAS O RANGE.IT WAS PUT OUT WITHIN 5 SECONDS WITH HANDFUL OF ROCKDUST.OCCURRED ON SHEAR 4' OUTBY FACE ON HEADGATE ENTRY WHILE CREW WAS PREPARING TO CHANGE OUT HEAD RANGE 3 R.M.
HE REPORTED TO HIS SUPERVISOR HE STRUCK HIS HARDHAT AGAINST THE MINE ROOF WHILE TRAMING #50 DIESEL PORTAL BUS TO THE SECTION. HE IS NOT ABLE TO IDENTIFY THE EXACT AREA.
HE WAS INVESTIGATING AREA SUSPECTED OF BAD TOP HAVING HEARD SOUND OF BREAKAGE. HE WAS FOUND UNDER A ROCK 4 INCH THICK 16 INCH WIDE BY 56 INCH LONG. THE AREA WAS SUPPORTED WITH 4 FT RESIN BOLTS ON 4 FT CENTERS. ROCK FELL BETWEEN.
HE WAS TRAVELING ON A JEEP MOVING FROM THE MOUTH OF 3 EAST TO MOUTH OF 5 SOUTH WHEN SOMETHING FLEW IN HIS LEFT EYE.

THE SHEARER WAS MAKING COAL RUN TO THE HEAD, GOING THROUGH A ROLL AT JACK 110. AN IGNITION WHILE THE HEAD DRUM WAS CUTTING THE SANDSTONE ROLL. THE FLAME WAS PUT OUT WITH A RESPIRAGLE DUST WATE R SPRAY HOSE.
AN IGNITION OCCURRED ON 5 DEV. EAST SECTION IN 5 LEFT BREAK WHERE THE MINER HAD REMOVED APP. 10' OF COAL. THE IGNITION BURNED THE OPER HEAD AND HANDS AND DISRUPTED THE FACE VENTILATION. THE IG NITION "BLEW" ITSELF OUT LEAVING A LARGE AMOUNT OF HEAT AND SMOKEY SOOT.
