An employee was troubleshooting a flow issue on a volumetric feeder. During the course of troubleshooting, an access hatch was opened and the feeder was jogged. At some point, the employee's hand entered a pinch point (rotary veins) before the feeder had come to a stop. The employee sustained partial amputation injury to 3 fingers.

EE had a recordable hearing shift during our annual on site mobile hearing test held on Nov 1st and Nov 2nd 2016. Employee was notified and required to go into the hearing facility to retest with in 30 days. The employee failed to follow up after several email and verbal reminders to schedule the off site hearing test and did not notify the appropriate person.

3/27/2015 in the AM Employee was taking a short cut and was scooting by a band saw table and caught his upper left leg (thigh) on a piece of steel. He states there was a minor poke and he did not think anything of it.
There no factors that contributed to this injury, The employee stated that he was walking at a normal pace.

Contractor was exiting a Bobcat when he stepped on to the face of the jack hammer and slipped. He rolled his ankle, resulting in an ankle sprain. No meds prescribed. No modified duty available.
There was a cable looped through grated walkway. Employee tripped but did not feel as though an injury occurred. a week later when his back hurt and he went to the doctor, this report was completed.
I was trying to loosen the motor base of the Silo 41 Pod Blower to remove the belts to test run the motor. Using a combination wrench, I was pulling on the wrench when I felt a slight tightness on my lower back. I was able to do my job. The next morning I felt pain in my lower back.
A rail car was shipped to Seattle Plant from Canada and arrived at Seattle Plant on July 31, 2013. When the rail car was spotted and opened a decomposing deceased individual was found inside. The coroner stated that the individual died several months earlier. He was not an employee of Lafarge.
Was playing basket ball when i was blocked by an opponent. Side stepped and my knee popped and crunched. I quit the game
Cutting 1/8th aircraft cable, a tooth caught on the blade, pulling finder into the blade, leaving a 1' cut. Contractor did receive stitches.
Miner complained of chest pains as he was walking to a job. Supervisor on shift called 911. The Seattle facility followed internal procedures that are in accordance with MSHA requirements and immediately reported the incident. Miner was having a heart attack. The miner was admitted to the hospital, underwent surgery and was released Sunday.
A contract employee was working on a refrigerated air dryer when a copper tubing line catastrophically failed. MSHA was already on site for our first regular inspection and an investigation is currently open. The contractor suffered a bruised abdomen, went to the hospital was prescribed a muscle relaxer and Ibuprofen.

Employee was suspected of having a heart attack when emergency personnel came to plant and took employee to hospital. She is under Short Term Disability leave and is under Dr. observation

EE was trying to close steerhorn and the feeder was jammed so he had to yank on it. The next day his back started hurting. Complained of back strain later and visited doctor on 11/16/2009.
Injured was unloading pet soil from truck. After unloading, He put both hands on top of dump gate and rocked the gate to empty out extra pet soil. The gare caught his left pinky and ring finger in a pinch point, causing two broken bones in fingers and receiving seven stitches.
Three employees were replacing the canvas on an air slide. EE was attaching a chipping hammer to an air line to begin work while the other two employees were working on the opposite sides. When EE was connecting to the air line, the bottom piece of the air slide fell onto his left foot. His foot is not broken, but swollen and painful and he required an air cast.
During routine chain maintenance for our shutdown an employee got his finger caught in between the drag chain and the tooth of the sprocket on the C6 drag line. The employee received 6 stitches and returned to work the next day. All equipment was locked out and non-operational at the time of the incident.

The natural gas boiler fired incorrectly resulting in a carbon monoxide (CO) emission in the main office. It was discovered by the presence of an odor. Immediately upon discovery all personnel were evacuated resulting in no illness or injury. Local fire department personnel assist in ventilation and source investigation.
Employee was on ladder watch when he ascended ladder to assist work partner with tarp. The ladder slipped and he fell from the 3rd rung.
Empl. turned ankle when stepping on mill ball. Reported that ball was not covered but just didn't see it. Incident reported as first aid because Empl. indicated he was fine and did not seek medical attention. On 8/31/08 employee was walking down a staircase and felt pain in ankle. EE went to doctor on 9/2/08 and was told not to return to work until diagnostic tests came back.
On 8/14/08 Employee reported a strained back which he linked to work he was doing on 8/12/08 when he was working on tail pulley on additive belt, using a pipe wrench to free belt so belt would track. This was an 8 hour job.
EE had received a hearing test that resulted with finding that the test results discovered a MSHA Record-able. In his right he has no response for 6000 Hz and 8000 Hz.
While working around briquetter, pulled guard up but did not secure prior to working beneath it. Guard fell, striking him on his hard hat (head).
An employee was attempting to push a glass tube through a hole in a rubber stopper (tube has a 90 degree bend in it). When pressure was applied to the tube, the tube broke at the bend. The employee's hand slid down and was cut on the broken glass tube that was sticking out of the stopper.

While working in Precip Hoppers employee broke out with hives. Until we received a report from his doctor on November 7, we were not aware that this was an industrial related illness.
Employee working with another employee rebuilding a slurry pump bearing barrel. Partner was using hammer & brass drift to drive shaft free while employee was holding bearing. Shaft shot out catching employee's hand, driving hand down to table resulting in laceration.
On 6/27/07 employee reported that at approx. 2:30 P.M. on the previous day, his eye started bothering him, which he thought was his contact lense, because he could not identify anything in particular he was doing that would bother his eye. The employee had been doing some jackhammering and may have gotten dust in eye without realizing it.
Employee was pushing Oxy/Acetylene bottle cart across the first floor mill room. He stepped on some mill balls, which he did not see, and lost his balance. He fell and the cart came down on his leg.
Employee was moving under trailer tipper to check a sensor. As he walked toward the sensor, he tripped on hydraulic cylinders. Employee fell to the ground bumping knee and shoulder on concrete pavement. His vision was impaired by his safety glases.

On December 6, 2006, EE reported soreness and stiffness which came about on the 4th when he was working in the precipitater and climbing in and out for a long period of time.
Employee lifted the shaft removal cap to place on coal mill roll assembly. While lifting, his back became strained.
Employee first reported incident on 11/3/06 but did not seek medical attention until 11/16/06. He reported bumping knee on steel plate while working on a pump.
Two employees were replacing return idlers on raw reclaim belt #3 when idler was released by fellow worker causing injured to hold it alone and he couldn't. The injured employee caught the idler with his legs and feet when he felt a sharp pain in his back.
On 10/11/06 employee reported to supervisor that his back was sore due to work he was doing on 10/7/06. Employee reported using chipping gun overhead to remove buildup of material in elephant trunk of #4 RR belt head pulley. On 10/12/06 at the end of the shift he went to doctor and returned to work on the 13th reporting that he had gone to the doctor.
While lifting a 94# cement bag, experienced pain and felt a pop in arm.
On 8/11/06 employee reported a sore lower back to his supervisor. He said the soreness was the result of pulling a cart of tools up several flights of stairs on the previous day.
While back flushing slurry pumps, tripped on grate and slipped in mud. First reported as first aid.

ON 1/16/06 EMP FIRST REPORTED TO SUPERVISOR THAT HE WAS EXPERIENCING DISCOMFORT WHICH HE BELEIVED WAS RELATED TO AN INCIDENT THAT OCCURRED THE PREVIOUS WEEK. HE WAS CHANGING A FITTING FOR THE HOSE ON A PORTABLE SUMP PUMP AND WHILE HOLDING THE PUMP IN A BEAR HUG, HE FELT PAIN ACROSS HIS CHEST.

WHILE HEATING SCREW GUDGEON WITH OXY/ACETYLENE ROSEBUD TORCH A FLASH BURN OCCURRED TO BOTTOM BACK RIGHT CALF. EMPLOYEE NOTED THAT OXYGEN LINE HAD BLED OFF WHILE AT ER, POSSIBLY OXY LEAK.
When leaving mens restroom to go to the lunchroom, contractor slipped and fell. Maintenance had just finished mopping floor at bathroon entrance & lunchroom floor was wet. There was no signage indicating wet floor.
Employee's wedding ring caught on the stem of a 2"" ball valve when he was stepping down off of a work platform. Did not seek medical opinion until 9/1/05.

Contractor cut tip of left middle finger with a table saw.
Emp stated he was replacing wear plates on aux collection drag trench & over exerted himself & experienced pulling in the groin area. Emp did not report the incident until 2/16/05 and sought medical attention on 2/17/05. Employee also indicated he had a similar injury 4 years ago which was not work related.
WHILE CHANGING OUT DSM SCREEN, EE WAS USING A HAMMER AND CHISEL TO REMOVE BOLTS. HAMMER IMPACTED LEFT HAND SPECIFICALLY THE INDEX FINGER
EE WAS WORKING IN KILN DURING SHUTDOWN. AS HE AND THE SUPERVISOR WERE EXITING THE KILN THERE WAS A CHAIN HANGING IN THEIR WAY. USING A SLEDGE HAMMER, THE SUPERVISOR CUT THE CHAIN DOWN. AS THEY PROCEEDED TO EXIT THE KILN A SECOND PIECE CAME DOWN, STRIKING EE ON THE ELBOW.

Waling into the office our visitor tripped on a slight rise in the sidewalk and fell. she was transported to swedish hosptial where x rays were taken and it was found that she had fractured the bone in her upper shoulder. She is not an employee of the plant, but was at the plant for training.
WHILE WALKING DOWN AN ELEVATED WALKWAY BY THE #2 FINISH MILL, EE STEPPED ON SOME CLINKER DEBRIS AND TWISTED ANKLE. THE FOLLOWING MORNING, EE EXPERIENCED SWELLING AND DECIDED TO SEEK MEDICAL ATTENTION.
EMPLOYEE WAS REMOVING COVER ON SCREW HOUSING. WHILE LOOSENING THE BOLT THAT WAS HOLDING IT IN PLACE, THE WRENCH HE WAS USING SLIPPED, HITTING HIS FRONT TOOTH AND BROKE THE CERAMIC CAP ON HIS TOOTH.
EMPLOYEE WAS DESCENDING A FLIGHT OF STAIRS. HE MISSED THE BOTTOM THREE STEPS AND CAME DOWN INJURING HIS KNEE. IT HAPPENED SO FAST THAT HE COULD NOT RECALL WHETHER HE HYPEREXTENDED OR HYPERFLEXED THE RIGHT KNEE.
ON 7/14/04 EMPLOYEE CONSULTED A DOCTOR FOR BACK PAIN WHICH HE STATED RELATED TO AN INCIDENT ON 7/1/04 WHEN HE WAS USING A PRY BAR TO CHANGE OUT A CLINKER COOLER FAN.
on 8/9 we received l&i claim for ee. ee states that he went to dr because of back pain. emp states that he has back problems which are unrelated to on the job incidents. however may have agitated his back . back on 6-9-4 when he was lifting a submersible pump out of a trench in the raw mill.
EE REPORTED THAT HE WAS PULLING ON A DRAG CHAIN WHICH HE SAID HE DOES ALL THE TIME. HE CAUGHT HIS MIDDLE FINGER BETWEEN THE CHAIN & SPROCKET CAUSING TEARING IN THE MEAT OF THE FINGER.
GRESHAM TRUCK DRIVER ENTERED PARKING LOT AT 1:00P.M. AND WENT UP TO LOADING PLATFORM TO OPEN HATCHES. TERMINAL OPERATOR HEARD HIM OPEN THE FIRST HATCH. WHEN HE DID NOT HEAR THE SECOND HATCH OP EN HE WENT TO CHECK AND FOUND THE DRIVER FACE DOWN ON TOP OF THE TRUCK INSIDE OF A SAFETY PLATFORM. HE WAS ASSISTED BY LAFARGE EMPLOYEES WHO PROVIDED EMERGENCY TREATMENT UNTIL PARAMEDICS CAME
EMPLOYEE SUBMITTED AN ACCIDENT REPORT AFTER THE FACT STATING THAT HIS THUMB WAS BOTHERING HIM. HE REPORTS THAT ON 01/22 HE WAS USING A SLEDGE HAMMER TO HIT THE LATCHING MECHANISM, THE DOOR SWUNG BACK HITTING HIS THUMB BETWEEN THE DOOR & HAMMER.

EE WAS INSTALLING A RETURN ROLLER ON A CONVEYOR BELT AND PINCHED HIS THUMB BETWEEN THE ROLLER AND BELT.
EE ATTEMPTED TO LIFT AN AIR CYLINDER ON HIS OWN RATHER THAN WAITING FOR ASSISTANCE FROM ANOTHER EE WHO WAS ON HIS WAY TO HELP.
LATE SUNDAY (7/13) MALFUNCTION IN AUTOMATED CONTROL SYSTEM ASSOCIATED WITH RAW MILL SUMP PUMP CAUSED SPILL ON MILL BLDG. GROUND FLOOR. EE WAS PASSING THROUGH AREA WHICH HE SHOULD NOT HAVE BEEN PASSING THROUGH FOR THE JOB HE WAS ASSIGNED AND SLIPPED ON THE WET FLOOR.
WHILE DOING CHAIN INSPECTION, EMP. REPORTED THAT ALTHOUGH SHE WAS WEARING APPROPRIATE PPE, SOME CKD MANAGED TO GET BEHIND THE SEAL OF HER RESPIRATOR. CKD MIXED WITH PERSPIRATION CAUSED A BURN ON HER CHEEK.
AS HE WAS PUSHING AND TWISTING A WECO VALVE INTO PLACE HE FELT A TEARING SENSATION IN HIS SHOULDER.
EE REPORTED ON 2-13-03, THAT HE WAS HAVING PAIN & MUSCLE TIGHTNESS IN HIS NECK AREA AND NOTICED A LUMP. HE FELT THIS WAS RELATED TO WORK HE WAS DOING IN THE RAW MILL ON 1-27-03. HE WAS PART OF THE CREW REPLACING MILL LINERS. WHILE LIFTING AND INSTALLING SCREW JACKS HE SAID THAT HE FELT A POP IN HIS COLLAR BONE AREA. ON 2-13 HE REPORTED THAT IT WAS BOTHERING HIM & WENT TO THE DR.
EE WAS PRYING ON A LINER HE WAS REMOVING IN THE RAW MILL. THE LINER RELEASED AND SLID DOWN. EE LOST HIS BALANCE AND FELL WITH THE LINER HITTING HIM IN THE RIGHT FOOT PINNING HIS RIGHT FOOT AGA INST ANOTHER LINER.

EE, WHO WORKS IN STOREHOUSE, WAS PUTTING BELTS AWAY. COULD NOT REACH OVER A BOX THAT WAS IN THE WAY SO DECIDED TO GO AROUND THE OTHER SIDE OF THE BOX. SOME SK SCREW CONTAINERS HAD BEEN PULLED AWAY FROM THE WALL & AS SHE WENT TO GO AROUND THEM, SHE CAUGHT HER FOOT & FELL.
EE WAS LIFTING A FIVE GALLON CAN WHEN HE REPORTED FEELING A PULL IN HIS BACK. ON 6-24-02 EE REPORTED EXPERIENCING PAIN AND REPORTED TO THE CLINIC TO HAVE IT CHECKED.
EE REPORTED THAT SHE WAS MOVING PALLETS INTO THEELEVATOR WITH THE WALKIE STACKER AND WHEN BACKING OUT OF THE ELEVATOR SHE HAD HER ARMS FULLY EXTENDED WHEN THE JACK CAUGHT ON SOMETHING AND STOP PED ABRUPTLY.
EE WAS REMOVING A PLUGGED 6"" PIPE. HE WAS CONCERENED THAT HE WOULD LOSE A PART OF THE FITTING INTO THE RAW MILL, SO WAS STEADYING THE PIPE WITH HIS LEFT HAND WHILE LOOSENING THE SCREWS THAT HE LD THE PIPE IN PLACE. WHEN THE PIPE CAME LOOSE IT DROPPED PINNING HIS HAND BETWEEN THE PIPE AND GRATE. EE REPORTED THAT HE DID NOT ANTICIPATE THE WEIGHT OF THE PIPE.

STANDING ON LADDER WHILE JACKHAMMERING GYPSUM BUILDUP. STEPPED DOWN OFF OF LADDER ONTO UNEVEN GROUND AND TWISTED ANKLE.
ON OCTOBER 10, 2001 EMPLOYEE WENT TO CLINIC COMPLAINING OF BACK PAIN. ALTHOUGH HE DID NOT REPORT A PROBLEM ON 9/20/01 HE FELT THAT THE REPETITIVE JOB HE DOES IN PACKING WAS THE CULPRIT.
EMPLOYEE REPORTED THAT HE RAN OUT OF CEMENT SO ADDED AIR TO THE SILO. HE OPENED A HALE TO SEE IFTHE CEMENT WAS FLOWING RELEASING CEMENT WHICH HIT HIM IN THE EYE.
EE WAS PACKING MASONRY CEMENT. TWO BAGS FELL AND EE TRIED TO STOP ONE. WHEN HE GRABBED THE BAG HE FELT A STRAIN IN HIS ABDOMEN.
EE STATES HIS SYMTOMS WERE THE RESULT OF AN INCIDENT THAT OCCURRED IN MAY 1999. WE HAVE NO REPORT OR RECORD OF HIS HAVING REPORTED AN INCIDENT. HE WENT TO THE DR BECAUSE HE WAS EXPERIENCING PA IN AND NUMBNESS IN HIS WRISTS.
EE WAS MOVING A PIPE HANGER IN THE COMPRESSOR ROOM. WHILE MOVING THE PIPE HANGER DUST BECAME AIRBORE AND A PARTICLE ENTERED HIS RIGHT EYE. THE EE WAS WEARING SAFETY GLASSES AT THE TIME OF THE INCIDENT.
ON 5-4-01 EE WAS WALKING UNDER A LOW BEAM. HE MISCALCULATED HEIGHT WHEN DUCKING UNDER AND STRUCK HIS HEAD ON BEAM. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 5-14-01 AT WHICH TIME HE COMPLAINED O F RESIDUAL PAIN.
EMPLOYEE WAS LIFTING A #94 BAG, WITH ASSISTANCE, UP AND OVER A HANDRAILING TO PLACE BACK ON CONVEYOR BELT. REPORTED FEELING SLIGHT PULL ON LEFT SIDE. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UN TIL 5/7/01 AT WHICH TIME HE WAS EXPERIENCING RESIDUAL PAIN.
WORKING ON AND OFF OF PLANK SPANNING THE BACKHOUSING OPENING. SHE WAS BREAKING UP HARD MATERIAL USING A PRY BAR TO BREAK UP A MASS OF CHUNKS. SHE WAS INVOLVED IN KEEPING THE CHUTE AND SCREW CL EAR OF CHUNKS. SHE COULD NOT IDENTIFY ONE SPECIFIC ACTION THAT CAUSED THE STRAIN. SHE DID NOT ECXPERIENCE PAIN UNTIL THE NEXT DAY.
EE WAS CLOSING LID ON LOADED RAIL CAR. HE TURNED TO LEAVE PLATFORM. INSTEAD OF USING STAIRS, HE JUMPED DOWN 3' TOLANDING. HE LIT WITH FULL WEIGHT ON RIGHT LEG BREAKING THE TIBIA BONE.
EARLY IN THE MONTH, EE WAS SHOVELING DEBRIS WITH A SHORT T-HANDLED SHOVEL. LATER IN MONTH EE EXPERIENCED SORENESS INLOWER BACK WHICH HE RELATED TO THE SHOVELING ON ABOVE DATE.
EE EXPERIENCED DIFFICULTY SHUTTING ELEVATOR DOORWHILE EXERTING MORE EFFORT WHEN PULLING ON STRAPTO CLOSE DOOR, SHE STRAINED BACK.
WHILE CHANGING PUMP MOTOR A BLEED VALVE HANDLE TO HOT WATER LINE CAUGHT ON COVERALL COLLAR, OPENING VALVE WICH SPRAYED HOT WATER ON EMPLOYEE'S BACK.
HE HIT FINGER WITH HAMMER WHILE REPLACING KEEPERPIN IN DRAG PIN.
EE WAS USING SLEDGE HAMMER TO REMOVE KEEPER PIN FROM DRAG LINK ON THE AUXILARY COLLECTION DRAG. WHILE HAMMERING PIN, HE STRUCK HIS FINGER WITH HAMMER.
WORKING ON AGITATOR WHICH WAS TORQUED OUT, IN ORDER TO RELEASE TORQUE, EE REACHED UP UNDER GUARDS AND PULLED ON BELT. BELT CAME FREE PINNING HISRIGHT INDEX FINGER BETWEEN BELT AND SHEAVE. A RA ZOR KNIFE HAD TO BE USED TO CUT BELT AND RELEASEHIS FINGER.

ALTHOUGH THE EMPLOYEE WAS BELTED IN THE RESULTING BOUNCE CAUSED HIS A WHIPLASH TYPE OF INJURY.
