Over-exertion in pulling or pushing objects

EE was grinding on a feed hopper. The electrical cord was caught on the ladder he turned to his right to pull some slack in the cord and felt a strain in his lower back.

While removing 20' x 2"" x 2"" steel tubing from the rack, a second tubing rolled from the pile. The edge of the tubing made contact with the employee's right forearm resulting in a 4"" laceration.

EE & co-worker were replacing & repairing track groups on the 330 excavator. In an attempt to align the lock slots with the slip fit pins to join the tracks together, EE tapped on the pin with a sledge hammer. The pins did not move. EE took a more forceful swing, the hit glanced off the pin toward the track pad where his hand was caught between the pad & the sledge handle.

His hearing is getting worse all the time. he wears ear plugs/muff and is in an enclosed cab. he has been a driller for many years prior to coming to work at pacific rock products all so it is not clear if the loss was caused by conditions at pacific rock products lll.

EMPLOYEE WAS HAMMERING ON A BLOW BAR. A PIECE OF METAL FLEW OFF AND STRUCK HIM BETWEEN THE RIGHT THUMB AND FOREFINGER. THE STEEL PIECE WAS REMOVED AND ONE STITCH WAS REQUIRED TO CLOSE THE WOUND.

EE WAS CUTTING A CONVEYOR BELT WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT HIS RIGHT HAND BELOW THE THUMB. THE LACERATION REQUIRED THREE STITCHES TO CLOSE.

Struck by falling object

EE WAS SHOVELING MUD UNDER THE SCALP BYPASS CONVEYOR. A PIECE OF WET MUD FELL FROM A RETURN ROLLER AND STRUCK HIM IN THE HARD HAT. THE IMPACT CAUSED HIS HEAD TO SNAP BACK JARRING HIS NECK. VIS ITED DOCTOR ON 9/22/03 AND DIAGNOSED WITH A NECK STRAIN.

EE WAS WELDING AND GRINDING ON A CRACKED CROSS MEMEBER OF A TRANSFER. HE HAD LIMITED ACCESS. HE WORE SAFETY GLASSES AT ALL TIMES HOWEVER HE DID NOT WEAR A SHIELD A TALL TIMES BECAUSE OF TIGHT ACCESS. THE NEXT MORNING HE FELT SOMETHING IN HIS LEFT EYE. IT WAS REMOVED AT THE CLINIC THAT DAY. RELEASED TO FULL DUTY.

EE WAS CHECKING THE BUCKET KICKOUT MAGNET ON HTE 988F. HE HAD CLIMBED ONTO THE ARMS OF THE LOADER TO ACCESS THE MAGNET. WHEN HE FINISHED THE WORK, HE CLIMBED BACK DON. HE PLACED HIS LEFT FOOT ON A BUCKET ARM AND WHEN HE TRANSFERRED HIS WEIGHT, HIS FOOT SLIPPED AND HE FELL. HIS LEFT SIDE STRUCK THE BUCKET ARM. IT WAS RAINING OUT AT THE TIME.

EE WAS IN THE PROCESS OF WELDING 1/2"" STEEL PLATE TO THE OUTSIDE OF THE 375 EXCAVATOR BUCKET. WHEN HE LIFTED THE PLATE INTO POSITION, HE RESTED IT JUST ABOVE HIS RIGHT KNEE. SEVERAL DAYS LATER THE KNEE SWELLED UP, BUT THERE WAS NO PAIN. HE VISITED A DOCTOR ON 10/30/02 AS THE KNEE WAS NOT IMPROVING. DIAGNOSED WITH POST TRAUMATIC PATELLAR FEMORAL SYNDROME OR WATER ON THE KNEE.

TWO EMPLOYEES HAD THE LID OF THE IMPACTOR IN THE RAISED POSITION TO CHECK OUT A PROBLEM. THE WEDGE SHAPED STOP BLOCK WAS IN ON ONE SIDE. VICTIM WAS ADJUSTING THE BLOCK ON HIS SIDE. HIS LEFT TH UMB WAS PINCHED BETWEEN THE TOP OF THE BLOCK AND THE BOTTOM OF THE HOOD. HE LOST THE THUMBNAIL.

EMPLOYEE WAS INSTALLING A NEW CURTAIN LINER IN THE CRUSHER. THE LINER SLIPPED AND PINCHED THE LITTLE FINGER ON HIS LEFT HAND BETWEEN THE NEW LINER AND AN ADJACENT LINER. AS HE PULLED HIS HAND OUT OF THE WAY, THE END OF HIS LITTLE FINGER WASCUT AND THE NAIL WAS DAMAGED. STITCHES REQUIRED TO TREAT THE INJURY.

HAUL TRUCK WAS DUMPING ROCK ON STOCKPILE. AS BED WAS BEING RAISED, TRUCK WENT OVER SIDE OF STOCKPILE, DOWN THE SLOP AND CAME TO REST AT TOE OF PILE. DRIVER WAS WEARIGN SEAT BELT. CONTRIBUTING CONDITIONS INCLUDE DRIVER PROCEEDING TO DUMP WAS TOO CLOSE TO EDGE AT AREA OF STOCKPILE WHERE BVERM HAD SLUGHED. THE TRUCK'S CATWALKS, HANDRAILS, CAB DOOR AND WINDOWS WERE DAMAGED.
