MAINTENANCE ON THE HORIZONTAL SHAFT IMPACT CRUSHER. HYDRAULIC LINE BLEW AND COVER TO CRUSHER CAME DOWN AND PINCHED THE COVER AT THE WAIST. THE INJURIES RESULTED IN A FATALITY.
