VICTIM WAS ELECTROCUTED IN AN AREA WHERE HE WAS NOT AUTHORIZED TO BE AFTER HAVING ENTERED THRU A WINDOW DOOR PADLOCKED. VICTIM WAS APPARNETLY ATTEMPTING TO RECOVER AN UNUSED PIECE OF HIGH VO LTAGE CABLE & AFTER CLIMBING TO THE CEILING OF THE BUILDING, FELL & CONTACTED ENERGIZED 4160V BUZZ BAR.

EMPLOYEE WAS SH2VELING SPILAGE FROM BETWEEN TRACK. EMPLOYEE WAS STANDING ONE OF THE RAILS WHEN HIS FOOT SLIPPED OFF THE RAIL AND TWISTED HIS LOWER BACK. EMPLOYEE 1 ST DAY OFF BECAUSE OF INJURY WAS 1-5-88. 3
EMPLOYEE WAS LO2SENING FROZEN TURN BUCLES AND WHILE USING A CRESENT WRENCH AS A LEVERAGE GAIN THE WRENCH SLIPPED OFF THE TURN BUCKLE AND EMPLOYREE STRAINED HIMSELF.EMPLOYEE SAID HE FELT A BURN ING PAIN IN HIS3GROIN AREA.(EMPLOYEE DIDN'T GO HOME EARLY OR MISS ANY WORK DAYS WORK BECAUSE OF THIS INJURY UNTIL 11-30-87.)
FALL OF ROOF AP1ROX. 75' OUTBY SPAD # 828 IN THE INTAKE ESCAPEWAY AT ANCHORAGE. 6' RESIN BOLTS WERE USED. FALL WAS TIMBERED TO REESTABLISH ESCAPEWAY.
TROLLEY POLE ON3LOCOMOTIVE CAME OFF THE TROLLEY WIRE AND WHILE STILL MOVING THE MOTOR MAN WAS REACHING FOR THE POLE WHEN THE POLE STRUCK A LOW SPOT IN THE ROOF AND CAME DOWN AND STRUCK THE MOT ORMAN ON TOP OF3THE RIGHT HAND INJURING THE FINGER NEXT TO HIS SMALLEST FINGER.
EMPLOYEE WAS RI8ING WITH MOTORMAN LISTENING FOR UOISE WHEN LOCOMTIVE JUMPED TRACE AND THE INJURED EMPLOYEE S BODY CARE DOWN HARD ON TO THE SEAT OF THE LOCOMOTIVE AND COMPLAINED OF BACK PAIN.
EMP WAS REMOVIN1 ROCKS FROM THE CONVEYOR BELT WHEN HE SAID HE TURNED & TWISTED HIS KNEE
WORKER WAS OPER5TING A LOCOMOTIVE WHILE TRYING TO PUT THE POLE ON THE WIRE WHILE STILL MOVING HE STRUCK HIS HEAD AGAINST A ROOF BOLT. EMPLOYEE WAS TREATED WITH FIRST AID.
FALL OF ROOF AP1ROX. 1350' INBY THE PITMOUTH IN THE INTAKE ESCAPEWAY AT ANCHORAGE. 4' CONVENTIONAL BOLTS WERE USED. FALL WAS TIMBERED TO REESTABLISH ESCAPEWAY.
OPERATOR WAS IN8TALLING RESIN ROOF BOLTS & WHILE ROTATING BOLT THE WRENCH CAME OUT OF THE CHUCK & STRUCK HIM IN THE FOREHEAD. THE CUT OF 1 1/2" WAS TREATED WITH FIRST AID BY SECTION EMT. THE E MP WAS THEN TAK9N TO THE HOSPITAL WHERE HE RECEIVED 8 STITCHES
INJURED EMPLOYE9 WAS DRILLING FACE WHEN HIS LEFTFELT PAIN IN LOWER BACK.
FALL OCCURRED I3 A THREE-WAY INTERSECTION WHICH WAS INITIALLY BOLTED WITH 4' BOLTS AND SPOT BOLTED LATER WITH 6' BOLTS FALL WAS APPROX. 15'X20'X6'. NO INJURIES PROPERTY DAMAGE, OR ENTRAPMENT. FALL WAS LOCAT9D IN THE 1ST RIGHT PANEL AT SURVEY STATION 1093.
EMPLOYEE GOT OU7 OF HIS VEHICLE UPON ARRIVING AT THE MINE SITE, AND SLIPPED ON ICE CAUSING HIM TO FALL.
INJURED EE STE1PED OFF OF LOCOMOTIVE ONTO A ROCK TWISTING ANKLE.
FALL OF ROOF IN3THREE-WAY INTERESECTION WHICH WAS ALSO A BACK-CUT LOCATED THREEE BLOCKS INBY SURVEY STA. 1064 IN #5 ENTRY OF 1ST RIGHT PANEL. FALL WAS AT ANCHORAGE-4' RESIN BOLTS WERE USED. NO EQUIPMENT DAMAG9 OR INJURIES TO REPORT.

INJURED EMPLOYEE WAS BLOCKING THE WHEELS OF AN EMPTY COAL CAR WHICH WAS OFF TRACK HE SIGNALED TEH OTHER MOTORMAN TO MOVE THE CARS AHEAD AND WAS APPARENTLY ATTEMPTING TO HOLD THE BLOCK WITH HIS HAND AND WHEN THE CARS MOVED HIS FINGERS WERE CAUGHT
INJURED WAS FILLING IN FOR AN EMPLOYEE OFF WORK. HE FAILED TO PROPERLY ADJUST HIS HAND BRAKE. THE TROLLEY POLE CAME OFF RENDERING THE ELECTRIC BRAKE INOPERATIVE AND A TRIP OF TEN EMPTYS WITH T HE LOCOMOTIVE OUTBY, TRAVELED APPROX. 200\ AND COLLIDED WITH LOADS AT THE TIPPLE. INJURED WAS ON THE LOCOMOTIVE AT THE TIME OF IMPACT AND CLAIMED HIS BACK WAS HURT.
