DISCOVERED IN SOUTH MAINS 001-0 SECT BETWEEN 4 AND 5 ENTRIES NEAR 22 STOPPING.FALL IS EXTENSION OF FALL REPORTED 6-9-84\ RESIN BOLTS WERE USED.SIZE-20\ X 20\ X 6\.NO INJURIES ,TRAVELWAY AND VE NTILATION NOT BLOCKED .AREA WILL BE TIMBERED OFF AT THIS TIME.

THE EMPLOYEE WAS MOVING PARTS OUT OF THE PARTS TRAILER AND PUTTING THEM IN A TRUCK TO TAKE TO ANOTHER LOCATION. HE PICKED UP TWO BELT PULLEYS FOR A STACKER AND STARTED OUT THE DOOR, AT WHICH T IME HE FELT SHARP PAINS IN THE LOWER PART OF HIS BACK AND IN HIS LEGS. HE WAS TRANSPORTED BY AMBULANCE TO THE HOSPITAL, BUT WAS NOT ADMITTED AT THIS TIME.
