A FALL WAS DISCOVERED ON THE NORTHEAST AMINS 002-0 SECTION AT THE #6 ENTRY #33 BREAKTHROUGH SPAD #2534 APPROX 2500\ AWAY FROM THE FACE AREA. THE FALL WAS ABOVE THE BOLT ANCHORAGE 4\ RESIN BOL TS WERE USED. SIZE: 20\ LG 5 1/2\ TO 6\ HT 20\ WD THERE WERE NO INJURIES & THE VENTILATION & TRAVELWAY WERE NOT BLOCKED THE FALL WILL BE TIMBERED OFF.
EMPLOYEE WAS PULLING ON A SLATE BAR TRYING TO MOVE A SCOOP CANOPY THAT WAS LAYING TOO CLOSE TO THE TRACK THE BAR BROKE AND HE FELL TWISTING HIS LEFT ANKLE HE CONTINUED TO WORK THE REST OF THI S SHIFT BUT LATER IN THE EVENIGN HIS ANKLE BEGAN TO SWELL AND HE WENT TO THE DOCTOR AND DISCOVERED A FRACTURE TO THE LEFT SIDE
