ON OR ABOUT 4-13-98, A PARTIAL FALL WAS ENCOUNTERED AT FAN ENTRY. APPARENTLY A SMALL AMOUNT OF DIRT AND ROCK FELL GRADUALLY DURING NEXT FEW DAYS THEN ON 4-17-98, FAN WAS CUT OFF COMPLETELY DUE TO FAN OPENING BEING COMPLETELY BLOCKED DUE TO ROCK FALL IN MAIN INTAKE.

MINE HAD BEEN IDLE. UPON CHECKING MINE, FOREMAN DISCOVERED ROOF FALL ON 1 PM, OCTOBER 10, 1995. REPORTED TO MSHA, JIMMY CLINE, AT 1:30 PM. EXACT TIME OF FALL NOT KNOWN.
