Employee was loading coal truck with loader when he backed over the edge of the coal seam onto the pit floor, which was not planed, causing him to be jerked around in the cab.
Employee was sitting in a 785D rock truck while it was being loaded. A larger boulder was dropped in tossing the truck from side to side causing the employee lower back pain.
Employee was getting out of dozer at the end of his shift. He stepped off of the push arm, slipped on the slick ground and fell.

Employee was greasing dive line when he slipped on the grease and fell off of the loader, hitting the ground.
Employee was in truck when a rock rolled across the truck bed, jarring him.
Employee was loading rock truck on North Side. He alleges he hit a hard spot on the right side of the bucket and was bounced upward from the seat.
Employee was going to check the fluid level in the dozer while it was heating. The cap blew off and hot liquid hit him in his face and left hand.
Employee was in rock truck. When rock was loaded, the truck rocked and he struck his elbow off steering wheel.

Employee stepped off the rear step of a mechanic's truck onto unleveled ground, rolling his ankle.
Employee was being loaded in the pit by the end loader when a rock that was getting loaded onto his truck slipped across the load and hit the other side of the truck bed.
Employee alleges he sprained/strained his back getting out of a dozer on 02/28/11. No company accident report was submitted. Employee began missing work on 03/28/11. Employee did not file Worker's Comp. claim until 04/12/11. Injury was questionable. Brick street ruled the claim compensable 05/03/11. Therefore, we are now reporting.

Employee was hitting a piece of steel in a rock truck bed with a sledge hammer, when a fragment of metal chipped off of the sledge hammer and lodged in his neck.

Employee was laying out shot pattern on drill bench. Mine break opened up under feet and the employee fell in.
Employee was starting equipment for the beginning of the shift. He climbed down off a dozer, stepped on a rock and twisted his ankle.

Tightening bolts with impact gun, gun kicked back off of bolt, shoving against right shoulder.
Employee was raising ladder up on PC1800. He got his finger caught between ladder & frame causing a laceration.
Employee was removing hose from 2500 Hitachi. The hose slipped out of employee's helper's hands & hit employee in the head.
Working on ladder while removing bed pins on 785C. While working with pry bar, lost balance and fell to ground. Working 6-8 feet off ground. Working on restricted duty - no time lost.
Employee bent over to lift a planitary cover off of a 988 loader when he felt a sharp pain in his low back & leg.
While using pry bar to remove air cleaner, bar slipped & hit arm on hose clamp for esther aid bottle, cutting left forearm. Received stitches.

EMPLOYEE WAS GETTING OFF DOZER. HE STEPPED DOWN TO THE TRACK WITH LEFT LEG, HIS RIGHT FOOT CAUGHT ON THE EDGE OF STEP & HE SAT DOWN ON RIGHT LEG. HE DIDN'T FALL, HE OVER-EXTENDED RIGHT KNEE.
EMPLOYEE WAS DRIVING UP HAUL ROAD WHEN HE STALLED OUT. HE WAS BACKING THE TRUCK UP TO GET OUT OF THE WAY AND WENT INTO THE DITCH.
EMPLOYEE WAS CHECKING OIL IN TRUCK. AS HE WAS COMING DOWN THE STEPS HIS FEET SLIPPED & HE FELL. WHEN HE FELL HE LANDED ON HIS HEELS, HIPS & BACK.

EMPLOYEE CHECKED OIL IN DOZER (D-9). HE WENT TO GET INTO CAB AND STRAINED HIS BACK WHEN HE TURNED.
EMPLOYEE WAS CHANGING FILTERS ON A HIGHWALL DRILL WHEN HE SLIPPED ON THE LADDER CAUSING HIM TO HIT HIS FINGER WITH A WRENCH.
Employee was fueling truck, when hanging up the fuel hose, it pulled apart causing fuel to spray into ee's face & eyes. Ee also claims that when the fuel pump began to spray him he twisted his back. Employee did not seek medical treatment for his back until 5/5/06. Protesting injury of the back.
Employee was hauling rock in a 777D. As he was being loaded, the 992G operator dropped a rock into the truck and it rolled off hitting the side of the truck bed. Jarring the employee inside the truck.
Employee was taking a plug out of a truck differential. When he was turning a bolt, he pulled a muscle.

EE HAD HEATED A FINAL DRIVE BOLT TO BREAK LOOSE WITH 1"" IMPACT. ALSO, HE HAD PUT BLASTER AIR TOOL OIL IN IMPACT. WHEN OIL CAME OUT WITH AIR PSI FROM IMPACT IT IGNITED AND BURNT HIS LEFT FOREARM, RIGHT HAND AND LEFT SIDE OF HIS FACE.

EE WAS RUNNING THE HITACHI 1200 EXCAVATOR LOADING THE TRUCKS WHEN THE HIGHWALL COLLAPSED INTO THE EXCAVATOR. HIS TRUCK WAS BURIED IN DEBRIS AND ROCK. IT TOOK APPROXIMATELY 1 HOUR TO DIG HIM OU T. EMT ON THE JOB TOOK EE TO THE HOSPITAL.
EE WAS PUTTING LUNCH BOX IN THE TRUCK & GOT PAPER TOWELS TO WIPE OFF HEAD LIGHTS. THE CAB FLOOR WAS SLICK WITH A CLEANING SOLUTION, WHEN HE STARTED DOWN THE LADDER HE SLIPPED & SLID DOWN THE LADDER. AFTER LOOSING FOOTING FROM LADDER HE CUT HIS LEG ON THE LADDER.
EE, BACKHOE OPERATOR, LOADING ROCK TRUCKS WITH ROCK HOE WAS CLEANING WALL- BUCKET SLIPPED OFF ROCK-JARRED EE, HE COMPLAINED OF BACK PAIN. EMT'S CHECKED HAYNES- EMT TOOK EE TO HOSPITAL TO BE EXAMINED.
ON 8/28/03 APPROX 7:30 PM, EE WAS DOING A P.M. SERVICE ON A 777D ROCK TRUCK. HE THREW A SMALL OIL FILTER FROM THE GROUND UP TO THE DECK OF THE ROCK TRUCK. THE FILTER ROLLED OFF THE TRUCK, HIT A STEP & BOUNCED INTO HIS FOREHEAD. HIS HEAD WAS CUT APPROX 3/4"". HE HAD STITCHES IN FOREHEAD.
ON 8-15-03 EE CALLED HIS SUPERVISOR AT HOME. HE STATED THAT AT QUITING TIME ON 8-14-03 HE WAS GETTING OFF A ROCK TRUCK, STEPPED ON GRAVEL AND HURT HIS KNEE. HE SAID AT THE TIME HE DID NOT THI NK HE HAD HURT HIMSELF. BY THE MORNING OF 8-15- 03 HE STATED THAT HIS KNEE WAS SWOLLEN.
LIFTING CUTTING EDGE FROM D-10 TRACTOR WHILE UNLOADING TRUCK.
EE HIT HEAD ON TOP CAB OF SUPERVISOR'S TRUCK AT QUITTING TIME. HE DIDN'T THINK IT WAS ANYTHING, SO HE WENT HOME. THAT NIGHT HE HAD A HEADACHE. HE CONTINUED TO HAVE HEADACHES & STATED HIS NECK BEGAN TO HURT. HE STATED HE WENT TO E.R. ON 4/17/03, BUT SAW HIS REGULAR ON 4/21/03, WHEN WE WERE NOTIFIED OF HIS INJURY.
ROCK TRUCK DRIVER TOLD FOREMAN AND SHIFT SUPT. THAT HE HIT ROUGH SPOT IN HAULROAD AND HURT HIS BACK. ROAD WAS WET, MUDDY, AND ROUGH. WEATHER-RAIN. HE WORKED THE REMAINDER OF SHIFT-1530-0030.

ON AUGUST 12, 2002, EE WAS FUELING THE D-10R DOZER 330, AROUND 3:15 PM. THE DOZER WAS NEARLY FULL OF FUEL & THE WIGGINS NOZZLE WAS STARTING TO SHUT DOWN. INSTEAD OF SHUTTING OFF, THE NOZZLE BL EW OFF OF THE COUPLING & STRUCK HIM IN THE LEFT SHOULDER.
EE WAS GOING TO FILL WATER TRUCK. WHEN HE TURNED THE 6"" PUMP ON IT HAD ICE IN THE LINE AND THIS CAUSED THE HOSE TO FLY OFF OF THE PUMP. WATER AND ICE HIT HIM AND KNOCKED HIM DOWN AND HE LANDED ON SOME ROCKS IN A DITCH.
EE WAS STARTING 777 CAT TRUCKS. HE HAD ALREADY STARTED A COUPLE OF TRUCKS AND WAS WALKING ACROSSA BANK TO START ANOTHER WHEN HE STEPPED ON A ROCK CAUSING HIM TO LOSE HIS BALANCE AND FALL.

EE WAS CUTTING BROKEN IDLER CAP BOLT OUT WITH A TORCH. A PIECE OF THE HOT SLAG WENT DOWN IN BOOT AND BURNT RIGHT FOOT. DATER OF ACCIDENT: 6-8-01. EE DID NOT SEEK TREATMENT UNTIL 6-12-01. HUMAN RESOURCE DID NOT RECEIVE NOTIFICATION UNTIL 6-28-01.
