HE WAS USING THE BACKHOE TO GET A ROLL OF MINE BELT. HE GOT OFF THE BACKHOE TO HOOK UP THE BELT AND THE MACHINE DRIFTED AND CAUGHT HIS FOOT BETWEEN THE BELT AND THE HOE BUCKET. THE MACHINE WAS CHECKED AND THE PARK BRAKE WORKED PROPERLY. IT APPEARS HE DID NOT ENGAGE THE BRAKE.

ROOF FALL IN THE INTERSECTION ONE BLOCK INBY THEFAN PORTAL, FALL IS 13'WIDE AND 15'LONG AND ABOUT 12'IN HEIGHT, NO EQUIPMENT OR PEOPLE WERE INVOLVED IN THE ROOF FALL.
