Mine No. 5: 859 MSHA citations Coal

Close Clearance and not keeping all body parts inside of mantrip. Employee crushed EE's middle and pointer fingers of EE's right hand when EE grabbed the canopy post to reposition self in the mantrip. The mantrip was traveling around a tight corner where clearance is close and the employees right hand was in a pinch point between the canopy post and the coal/rock rib.

Employee was setting timbers in the #2 entry and was picking up a timber and felt a pull in groin area worked for anther hour and said it was burning employee was took outside and went to the ER.

Employee was operating miner on 004 section EE was cutting coal in the 7R crosscut EE was cutting the first sump in on the left side loading a shuttle car when a piece of bottom rock 24"x30" flipped out from under the ripper head and struck EE in the lower right leg.

Employee was using a 1/2" ratchet to take out 5/8 bolts on continuous miner ratchet slipped causing EE to fall. Was complaining of lower back hurting. Employee has worked up till 3/24/2020 and EE's back started hurting again.

Employee was building a stopping felt pain in lower back and left leg.

Employee was loading tool box from tool cabinet to the mantrip. Picked up tool box from cabinet and turned around to load on mantrip and felt a sharp pain and burning in lower back.

Employee was exiting the loader and twisted left foot when stepping off of loader.

Employee was hanging miner cable on 002 section and pulled muscle in right shoulder.

Employee was operating a 488 scoop on 003 section hit a hole in the road and jarred lower back.

Employee was moving a miner cable out of the roadway while ee was pulling something pulled in right arm.

Employee was filling the continuous miner up with oil and when ee was opening up another can hand slipped cutting left index finger. The metal can had a rigid place where ee cut with a knife.

Employee was changing rollers on the slope belt went to pull old roller out from under the belt and hurt back.

Employee was working on 003 section opening a box of rags with a utility knife and cut left thumb. EE received 11 stiches in the thumb.

Employee was pulling a curtain across the section to hang and tripped on curtain fell on left hand and sprang thumb

Employee reached down to grab a hold of a curtain & started pulling it towards the face & felt a pain in stomach. The employee had helped hang a miner cable before moving the curtain. Mine wasn't notified till 10/9/2018

injured employee was stacking (6x6x36) crib blocks for section tail piece and mashed EE's left pinky finger had to have 6 stitches put in EE's pinky finger.

The injured employee was operating a Johnson 4wheeler when EE stopped to get off. EE stepped on lip and twisted the right knee. The injured employee didn't start missing work till 10/2/2018.

Injured employee pulled a muscle in lower back while lifting a I-beam to set head drive on. Employee didn't start missing work till 7/13/2018.

While making a belt move the injured employee stepped across the low-low and right knee gave out. The belt drive was locked and tagged out at the time of the accident.

The EE was lifting on a belt rail on the 001 section #4 belt to install belt structure and pulled a muscle in lower back. The accident was reported on the 4/20/2018.The EE started missing on the 4/23/2018.

Was using a hammer and it slipped and hit the employee over the right eye

Employee was making a cable splice and cut right leg

Employee was crossing a shuttle car cable when the cable jerked up causing the employee to fall on right arm and head.

EE carried two five gallon cans of hydraulic oil from power box to the shuttle car. When EE sat them down, EE felt pain in lower right side back and heard popping noise.

Unintentional roof fall in unused entry. Fall location is 1st West mains inby spad 325. Fall measured approx. 16'W x 70'L x 5.5' high. Area was supported by 48" resin bolt. No injuries, no equipment damaged & ventilation not interrupted. Fall was dangered off. No plan of moving the fall at this time. Time of the fall was approx. Fall was discovered later during day. Inactive mine.

An unintentional Roof Fall occurred in unused entry. The fall location is 1st West Mains, inby spad 311. The fall measured approximately 19' wide X 20' long X 5' high. Area was supported by 48" resin bolt. There was no injuries, no equipment damaged, and ventilation was not interrupted. Fall was dangered off. There is no plan of moving the fall at this time.

The continues miner was shearing a corner when the cutter head struck a rib bolt breaking the head off and sending it across the mining machine striking ee on his upper lip resulting in a laceration that required 9 sutures.
