EE was found unconscious at approx. 10:00 am. Quarry foreman was notified, went to scene & called 911 & Asst GM. EE regained consciousness by time paramedics arrived. Due to his medical history, was life-flighted to Ruby Memorial for further review. It was determined he suffered a seizure, causing his unconsciousness.
Rock bridged in chute from 6x20 to Belt #3. Must clear from chute on bottom side. When rock let loose, EE was unable to get his hand out before rock smashed fingers.
Employee caught left index finger and glove on drill bit. Before drill was shut off it twisted his glove and hand around the bit and amputated his index finger at mid-knuckle.
Dismounting hoe & stepped onto stumped mound of dirt. Slipped off stump & his left leg caught between stumped dirt mound & hoe track. Taken to ER. MRI showed nothing but was to have physical therapy & light duty till cleared by Dr. Returned to work under company's Back to Work Program on 7/17/07 for light duty at scalehouse.
