Caught in, under or between a moving and a stationary object

EE was riding on the cx2 scoop being operated by other ee was cleaning #1 entry when he backed op and swivled the scoop. His foot got caught in the operators side of the center section. Crushing his foot causing a possible break to the bones in his foot.

Struck by falling object

WHILE INSTALLING STRAP WAITING FOR OTHER MAN TO SWING HIS BOOM, AS HE WAS HOLDING THE STRAP A PIECE OF SLICK SLATE FELL THROUGH OPENING IN HALO RING, HITTING EE ON THE RT. HAND. ROCK WAS 2'X 2' & 1 1/2""THICK. EE HAD A SMALL CUT & SWELLING ON LEFT SIDE OF HAND BEHIND THUMB.

Accident type, without injuries

Roof condition occurred in an area where several cracks were present. When a cut was taken out of the crosscut to the right it tied the cracks together in a triangle. The top started working in this area. A crib was built in the intersection. In time the laminated roof broke out around the crib.

Over-exertion in lifting objects

EE BENT DOWN TO PICK UP A CRIB BLOCK AND PULLED A MUSCLE IN LOWER BACK

EE was tramming the miner across the section. He had stopped to hook a pull rope on boom of miner, when he accidently pinned himself between boom of miner & rib.

While tramming the miner, ee was struck in top of the head by a piece of draw rock. Rock measured approx. 10'L by 12"" thick.

EMPLOYEE WAS BOLTING TOP. THE WRENCH SLIPPED OFF THE DRILL STEEL STRIKING HIM ON THE HAND.

Employee was bolting top when a loose piece of coal struck him in the eye.

EE WAS BOLTING TOP, DRILL POT WAS STOPPED UP. EE BENT DOWN TO UNSTOP DRILL POT WITH A PIECE OF WIRE WHEN A PIECE OF ROCK STRUCK HIM ON THE SIDE OF THE HEAD, CUTTING HIS HEAD.

EMPLOYEE HAD FINISHED BOLTING #7 ENTRY ON MAINS. AFTER BOLTING FACE HE BACKED OUT OF FACE ABOUT 30' FOR THE BOLTMAN ON OFF-SIDE TO SPOT A BOLT. EMPLOYEE WALKED AROUND TO OFF-SIDE OF BOLTER TO HELP OFF-SIDE BOLTMAN WHEN A LOOSE PIECE OF ROCK FELL FROM A SURFACE CRACK, STRIKING HIM ON THE LEG.

Area was on a point, roomed on 50' x 50' centers several cracks present. area bolted with 4' fully grouted bolts on 4' carters with straps. a wedge shaped piece of sandstone 16' long x 14' wide x 7' thick fell out on miner. fall will not be cleaned up except to move miner. section will be moved 700' outby.

Fall to the walkway or working surface

EE had been making a belt splice. After completing splice and putting up his tools, ee slipped on slick mine bottom catching himself with his hands, stoving his pointer finger on left hand.

Fall is 20' x 20' x 5' thick, bolted with 4' & 6' fully grouted resin & roof straps. Area was cracked up bad on development and upon completion of mining the area was breakered off. Developed May 10th, 04. Fall is 800' from active section near spad 152. All areas are breakered off. Fall will not be moved.

Fall is 8' wide, 65' long, 5' thick, bolted with 4' resin and roof straps. Area cracked in several places on development 5-20-04. Fall near spad 174. Fall is breakered off and will not be moved.

EMPLOYEE WAS PREPARING TO INSTALL A ROOF STRAP WHEN A PIECE OF ROOF 25""X 25""X 3""THICK, FELL OUT BETWEEN BOLTS, STRIKING HIM ON THE HEAD.

EE WAS SHOVELING AROUND STACKER TAIL ROLLER, WHEN A LOOSE PIECE OF ROCK BOUNCED OFF BELT STRIKING EE ON THE LEFT SIDE OF THE FACE AND FOREHEAD.

INTERSECTION IN #4 ENTRY L.O.B.- THE FALL STARTED ON OUTBY CORNER AND FELL FOR ABOUT 35' INBY TOWARDS FACE AND BROKE OFF. TOP WAS SOLID WHEN BOLTED ON 2-14-04. NO WATER. FALL SHOWS SIGNS ""NOW"" OF WATER AND CRACKS WHERE FALL OCCURRED.
