We had a roof fall measuring 4 to 5 feet thick X 19 to 20 feet wide X 70' in distance. This fall is a continuance of a fall reported on 1/5/12 on the 2nd North West Mains @ Spad #1341 in the #4 and #5 entries. This fall does not impede ventilation or travel-The fall will not be cleaned up-There were no injuries.

A roof fall occurred in the old works (in rooms) in 5th Right off 2nd North West Mains at spad No. 1364 No. 6 Entry. The fall is approx 30 to 35 ft long, 20 ft wide and 4 1/2 ft to 5 ft thick (Ht). The fall will not be cleaned up.

Employee was removing gob from #4 tailpiece. The gob was coal and rock mixture, a piece of rock fell thru and rolled onto right index finger, smashing the employees finger.
