EE was performing site maintenance, driving a skid steer, when EE went to exit EE felt pain in EE's lower left back. Employee sought medical treatment on 8/18/25, where EE was ordered to do PT. Reporting within 10 days.

Employee was moving multiple oak hardwood blocks from ground level to a transport crate. We were notified on 7/17/24 that the Employee sought medical treatment and was diagnosed with a hernia.

Employee was lifting a drill steel to insert it into the drill's hammer. As employee was lifting the drill steel EE felt pain in abdomen. EE was diagnosed with a hernia & surgery has not been scheduled yet.

EE was shoveling stone and felt pain in right knee. Initial diagnosis was sprain and he did not receive any restrictions. Pain continued. EE's new diagnosis is a medial meniscus tear. Surgery is on 3/19/12. Note: EE was laid off for end of season after his shift on 12/2/11.
