HYDRAULIC CYCLINDER PLUNGED OUT AND CUT LEFT HAND RING FINGER. CYLINDER WAS BEING REPACKED AT TIME OF ACCIDENT GRINDER HAD SLIGHT DOWNWARD ANGLE CAP BOLTS WERE REMOVED CYLINDER ROD SPLIPPED FR OM BORE PINNING FINGER BETWEEN ROD AND BUCKET.
