OAKFIELD: 50 MSHA citations Metal/Non-Metal

Employee works at the Oakfield Quarry as a loader operator. While banding a pallet in the quarry, the employee cut the right thumb and index finger. The banding did not spool as freely as the employee was expecting and when they pulled harder on the banding, it slipped through their hand cutting it.

Employee works outdoors in an above ground quarry operating a loader. While banding a water pump hose, the banding cut EE's hand. Employee sought treatment for the laceration.

Employee was opening a stone; he lost his grip on the stone. The stone fell onto the one behind it pinching the employee's left thumb. Restricted duty assigned.

Employee was setting stone down on pallet when he felt a sharp pain in his back. Restricted duty assigned.

Employee was using hammer and tracer to open a stone. A piece broker off and hit the employee in the groin causing a contusion. Employee was given two prescriptions.

On 4/22/10, employee was lifting 60-70 lb stones off pallet; on 4/23/10 employee helped pack 100 holes for blasting. Employee's back hurt over the weekend. Employee did not report injury until 4/26/10. He did not seek treatment until 4/30/10. Restricted duty assigned.

Employee was walking across driveway to the break trailer when he slipped on ice and fell landing on his right shoulder. Restricted duty assigned. **Employee did not report the injury until 1 2/15/2009**

Employee slipped on the ice and hit his right elbow on the stairs of a Loader. Diagnosis: Elbow Contusion. Restricted duty assigned. **Employee did not seek medical treatment until 3/4/08**

Employee was lifting stone and felt a pain in his groin. Employee did not seek treatment until 1/5/07 - Restricted duty.

Employee was hurrying around truck. He slipped on a patch of ice. He grabbed the mirror bracket, his wedding ring got caught, causing laceration and possible ligament or tendon damage.

Employee lifted a rock off ground and felt a pain in his back. Lumbar Strain - Restricted Duty.

Employee went to catch a stone falling off a pallet and felt pain in his back. The employee did not report the injury until 11/29/06. He did not seek treatment until 11/29/06. Light duty - low back strain.

Employee was opening stone with a tracer. A piece of metal broke off and became lodged on the employee's arm. Restricted duty until surgically removed.

Employee was stacking a stone on a pallet. The stone under the one he stalked broke and a piece on the stone hit the employee in the leg requiring stitches.

Employee was carrying stone to put in crate; he rolled his ankle. Sprained ankle - restricted duty.

Employee was stacking stone on pallet. Finger got caught under the stone. Laceration/possible fracture.

EMPLOYEE WAS WALKING & TWISTED HIS ANKLE, HE SPRAINED HIS ANKLE. EE DID NOT SEE A DR. UNTIL 6/7/05.

EMPLOYEE WAS LIFTING ROCKS OFF GROUND & PUTTING ON PALLETS. HE FELT PAIN IN HIS KNEES (TENDONITIS). EMPLOYEE DID NOT REPORT INJURY UNTIL 6/2/05.

Employee was stacking stone on pallets. That night felt pain in his wrists. Wrist strain (both wrists).

Employee was stacking stone on pallets and felt pain in his lower back.

Employee drives a loader. He started complaining of pain going from lower back down his right leg. He did not go to doctor until 7-22-04. Diagnosis siatica - recurring from old injury.

EMPLOYEE WAS STACKING STONES IN A CRATE. HE DROPPED A STONE ON HIS FINGER & FRACTURED RIGHT MIDDLE FINGERTIP.

EE WAS WALKING FROM QUARRY TO BREAK ROOM. HE SLIPPED ON ICE AND FELL, FRACTURING HIS RIGHT WRIST.

AEE WAS PULLING OUT HOSES TO SET UP WATER PUMP HE SLIPPED IN THE MUD AND FELL, LANDING ON HIS RIGHT SIDE RIGHT ELBOW JOINT STRAIN

EMPLOYEE WAS CARRYING STONES & STACKING THEM ON A PALLET. HE TRIPPED OVER A STONE ON THE GROUND, CAUSING HIM TO DROP THE STONE HE WAS CARRYING ON HIS HAND. CRUSHED RIGHT INDEX FINGER.

EMPLOYEE WAS USING A HAMMER TO OPEN A STONE. HE FELT A PAIN IN HIS RIGHT SHOULDER BLADE. DIAGNOSED WITH A ROTATOR CUFF TEAR.

EMPLOYEE WAS PICKING STONES UP OFF THE GROUND AND PLACING THEM ON PALLETS WHEN HE STARTED HAVING LOWER BACK PAIN.

EE WAS CARRYING A 20-30LB. ROCK IN A WET WORK AREA WHEN HE TRIPPED AND FELL BACKWARDS. HE LANDED ON A SPREAD PILE OF ROCKS; SPRAINING HIS LOWER BACK.

EE STARTED COMPLAINING OF A SORE BACK APPROX. 1-2 WEEKS AGO. HE THOUGHT IT HAD TO DO WITH BACK SURGERY HE HAD A FEW YEARS AGO. HE WAS DOING STRETCHING EXERCISES AT A TRAINING WHEN IT BECAME WORSE. DR. DIAGNOSED A SEVERE LUMBAR STRAIN.

EE WAS PICKING UP ROCKS ON GROUND AND PLACING THEM ON PALLETS. STARTED COMPLAINING OF A SORE BACK.

EE WAS PICKING STONES UP OFF THE GROUND AND PUTTING ON PALLETS WHEN HE FELT A BURNING SENSATION IN THE BACK OF HIS NECK AND LEFT SHOULDER AREA.

IMPROPER AND REPETITIVE LIFTING OF ROCKS CAUSED ACUTE AND CHRONIC BACK STRAIN.

EE WAS PICKING UP STONES AND PLACING THEM ON PALLETS WHEN HE FELT A PAIN IN HIS RIGHT WRIST. EE WAS DIAGNOSED BY DOCTOR AS HAVING A SPRAINED RIGHT WRIST

THE EMPLOYEE WAS LIFTING A STONE FRON THE GROUND WHEN HE FELT A PULL IN HIS GROIN AREA. RIGHT GROIN STRAIN.

WHILE CARRYING A STONE. EE TRIPPED DROPPING THE STONE ON HIS RIGHT THUMB.

FINGER WAS CAUGHT BETWEEN 2 ROCKS.

STOOD UP A LARGE PIECE OF STONE AND IT FELL INTO HIS LEFT THUMB AND PALM.

REPORTED ARM WAS GOING NUMB, DID NOT KNOW WHY.

EMPLOYEE PICKED UP STONE AND TWISTED WRONG, RESULTING IN A SORE BACK.

HIS RIGHT WRIST BECAME SORE WHILE LIFTING STONE.

EE WAS PUTTING A ROCK DOWN AND CUAGHT HIS FINER BROKE.

EMPLOYEE TRIPPED OVER A PALLET AND FELL LANDING ON HIS WRIST. HE WILL BE DOING LIGHT DUTY UNTIL RELEASED FOR FULL DUTY.

EMPLOYEE SLIPPED ON A SMALL ROCK IN MUD AND TWISTED HIS LEG PULLING A MUSCLE.

EMPLOYEE CAUGHT HIS FINGER BETWEEN TWO STONES CAUSING HIS FINGERTIP TO BREAK. HE WAS TREATED AT BELOIT MEMORIAL HOSPITAL ON 4-8-95. HE INFORMED BUECHEL STONE CORP. ON 4-10-95 THAT HE HAD QUIT. NO RESTRICTIONS WERE PLACED ON HIS WORK ACTIVITY TO THE BEST OF OUR KNOWLEDGE.

EE FELL DOWN THE STEPS. HE TORE HIS LIGAMENTS IN HIS LEFT KNEE.
