Employee lost balance and fell cutting hand on a piece of metal grating causing a laceration in right hand that required stitches. Employee was released back to work same day with no restrictions.

employee has swollen hand and wrist, said it was caused from using a rubber mallet for several hours on shift. Went to the Dr but didn't report until several days later, went back to Dr got a full release back to work.
Employee fell while doing maintenance on wash plant and landed on Right hip causing pain, employee finished out shift, returned to work the next morning and left in afternoon to go see Dr. Employee has a deep tissue bruise and is being treated by a Physical Therapist.
EMPLOYEE WAS DOING PLANT ROUNDS AND STEPPED ON UNEVEN GROUND FALLING AND FRACTURING RIGHT FIBULA
Employee drank out of an opened dirty water bottle that was on the floorboard of a service truck. EE said EE realized after EE swallowed a small amount that it wasn't water.

Working conditions were good. Miner was is seated stable position performing operation. Miner lost grip/control when cut-off wheel grinder kicked back below waist area and rebounded off hopper/structure striking miner along cheek resulting in laceration. Miner does not recall what caused kickback. Miner was wearing safety glasses during cutting.
