THE AREA OF THIS FALL HAD SEVERAL HILL SEAMS RUNNING PARRELL WITH THE #1 ENTRY. THE ADVIRSE CONDITION SO FTHIS AREA HAD BEEN OBSERVED BEFORE THE FALL OCCURRED AND TIMBERS AND THE NECESSARY ST EPS TAKEN TO PREVENT EMPLOYEE FROM ENTERING THE AREA HAD BEEN DONE BEFORE THE AREA FELL. THE FALL OCCURRED DURING ON OFF SHIFT. THIS AREA IS 20' INW IDTH 25' LENGTH & 6'TO7' IN HEIGHT.
EMPLOYEE WAS HELPING REMOVE TIRE FROM A 482 SCOOP WHEN THE TIRE FELL OFF STRIKING THE EMPLOYEE ON THE RIGHT ANKLE.
