Fly rock from a blast flew over stockpile and hit employee in the leg. Rushed to nearest hospital and diagnosed with broken rt. leg.

Employee was trying to hoist himself up to look at screen deck which had been shut down but was still slowing down. He stepped on the guarding of shaker shaft. The rubber part of guarding gave when stepped on and his left foot went through to the shaft which had not come to complete stop yet. The turning shaft pinched his foot causing a cut on the bottom without cutting his boot.

EE PLACED A PRY BAR ON AN I BEAM ABOVE WORK AREATHE PRY BAR FELL OFF AND HIT HIM IN THE HEAD. LACERATION REQUIRED 4-5 STITCHES.
