While flipping gates on the screener, bent over to reach for equipment resulting in a low back strain.

Mine was not operating when the quarry manager was notified that there was a brush fire headed toward an inactive area of the mine. Fire was extinguished prior to getting to active mining area. A decommission water truck was damaged during the fire. There were no miner fatalities or injuries.

A bee came in through the window and stung employee on right calf. A couple of days later sting became infected.

Employee was trying to clear a rock that was stuck in the jaw. He was using a wedge attached to a chain, standing on the platform above the jaw. The wedge bounced up and as it fell he was holding onto the chain and the weight pulled his shoulder.
