Working on the Komatsu he complained of back pain.
While carrying an empty container, he slipped and sprained his right ankle.
He was at Plant III helping the welder, he had to pass the belt of the crusher to the stairs he lost balance and fell on the belt hitting it with the right side of his back, causing a bruise.
The miner alleges that while cleaning plant III with a machete he cut himself on the left knee, he got four stitches on it and was given many days of rest.

He (the injured) was helping the welder providing him metal rods to weld a part. When the welder finished he started to clean the part and from the dirt that came out, a piece of approximately 1/2"" hit him in the right eye causing him a laceration.

While adjusting the bolts of the grinder in Planta I, the bar slipped and he lost his equilibrium hitting his left hand against the conveyor & injuring small finger of left hand.
While tightening the bolts on the rollers of the excavator with a cup, a handlegrip and a piece of tubing, the piece slipped and hit him on the little finger of the left hand.

While cleaning under the grinder in Plant #2, the grinder filled up with rocks and they fell near the screens hitting him on the head. A rock weighs about 8 pounds. The impact of the rock tore the straps of the helmet and resulted in a bump that swelled up on the head.
While repairing the transmission, he took a piece out of the clutch and the transmission moved. It hit him on the muscle of the right leg causing pain.
After placing the curtain ring in the axle's end cover, he hammered it. A fragment of the metal flew into the left leg muscle and embedded causing an injury.
While cleaning underneath the strap of plant #4, a rock slipped hitting EE on the left hip.
While storing a soldering machine, the machine slipped and traumatized the left hand.

WHILE POLISHING INSIDE THE FEEDER (HE HAD A SAFETY MASK & GLASSES) HE FELT THAT SOMETHING FELL INTO HIS LEFT EYE. HE WASHED HIS EYE OUT BUT THE ANNOYANCE CONTINUED.
While loosening a screw on machine #9 and pulling the key, it (the key) slipped and became tangled int he machine hitting EE's thumb on his right hand. (Conversation with ee had to have stitches on his thumb and missed no work but did have restricted duty. He supervisor work during his restricted period.)
Every time that Employee begins to work he verifies/checks that one of the loose screws on the excavator is tightened. When he was climbing down he slipped on the small ladder on the excavator and the ring on his finger on his left hand struck the ladder causing a small cut on his finger.
THE LATERAL (SIDEBOARD) HAD COME OFF THE PLANT CONVEYOR BELT AND COWORKER TURNED OFF THE CONVEYOR TO LOCATE THE LATERAL. EE PROCEEDED TO ACCOMMODATE THE LATERAL (PICK IT UP & POSITION IT ON THE BELT) BUT EE DID NOT NOTICE THAT THERE WAS A STONE FALLING. THE STONE STRUCK EE BETWEEN THE NAPE OF THE NECK, HEAD AND SHOULDERS.
While driving a truck, ee felt a strong pain in his left shoulder and he has not returned to work.

The employee was cleaning under the conveyor of Plant II, a metal bar fell off the strap (conveyor) & bounced off EE's rt. chin causing a cut on the outside & inside of his lip. (Rt. mandible traumatic wound).
While cleaning the plant 3 breaker (crusher) a piece of 2 by 4 wood fell, hitting the index finger of his left hand.
He alleges that the rash he has on his legs, arms and flanks (the outer side of the upper part of the thighs) was produced due to his work with the company.
ENGLISH TRANSLATION: EE WAS MOUNTING A PINEAPPLE TO THE CONVEYOR WITH A COWORKER IN THE PLANT. THE PINEAPPLE FELL, CAUSING EE TO RECEIVE THE FORCE OF THE IMPACT ON HIS WAIST. THIS OCCURRED AS THEY TOOK A HOLD OF THE PINEAPPLE.
Working in plant II placing lateral, pinched left hand causing laceration to 3rd, 4th, & 5th fingers.
EE ALLEGES AS HE WAS GETTING OFF THE MACHINE HE STEPPED ON A SHARP ROCK & SPRAINED HIS LEFT FOOT. HE FELT PAIN AND THAT SOMETHING HAD BROKEN IN HIS LEFT KNEE. EE HAD SURGERY ON THAT KNEE & HAS NOT RETURNED TO WORK. NOT EXPECTED TO RTW FOR AT LEAST 3 TO 4 MONTHS.
WHILE INSPECTING THE CATERPILLAR TRUCK 7769B#4 (WHICH WAS NOT RUNNING AT THE TIME), EMPLOYEE, WHO IS A MECHANIC, WAS STRUGGLING WITH THE BATTERY CABLE. HE ASKED THE DRIVER TO START THE ENGINE WHICH CAUSED THE BATTERY ACID TO SPLASH ON HIS FACE.
EE ALLEGES THAT AS A CONSEQUENCE OF THE CARBON DIOXIDE GASES AND DUST PARTICULATES POWDER FROM THE TRUCK THAT THIS CAUSED HIS ASTMAS TO FLAIR UP. HE HAD NOT RETURN TO WORK.

HE WAS REMOVING A CHAIN AND A PIECE BROKE OFF, CUTTING HIS FOURTH FINGER ON HIS RIGHT HAND.
SOEMTHING FELL IN HIS LEFT EYE WHEN HE WAS CUTTING WITH AN ACETYLENE TORCH.
EE WAS TAKING A BEAM TO THE WELDER. FROMT HE BEAM A LUMP OF DIRT FELL INTO HIS EYES.
