Track drill operator during operation, finds the drill bit has been obstructed with mud, EE unscrews the drill bit to use a thin shaft to pierce the obstructed drill bit shaft, but was not aware than in the process of removing the drill bit, the shaft also unscrewed. While cleaning the drill bit shaft, the shaft came undone and pinched EE's right index finger.
The employee received instructions for positioning and the order in which they were to perform drilling pattern. Employee positioned the outrigger in a position where the weight distribution was against them and when they adjusted position, the machine shifted causing the employee to suffer a bruise to left toe, left ankle and right shoulder

The employee was a passenger in a golf cart and while going up hill, the cart lost power and when the brakes were applied, they failed. The employee jumped from the cart and when EE landed on the slanted ground, EE received a minor blow to the head, in which most of the impact was taken by EE's left hip, lower back and receive lacerations to both elbows.

A welder had an excavator bucket held upright by a metal leg. To lay the bucket down, EE attached a chain to it and removed the metal leg. As the chain lifted, the bucket laid itself towards the welder and caught EE between the bucket and a table that had a welding machine on it, injuring EE's left abdomen and left leg.
The employee indicates that while they were doing preventive maintenance (oil and filter change) on the John Deere 350D when they released the engine protector cover it fell on their left hand
The employee indicates that on Saturday, June 24, while EE was cleaning the tools with the pressure machine, EE injured left hand with the pressure of the water.
The employee reports today, june 16, 2023, that on Monday, June 12, 2023, while working on the installation of a water pipe on the Cantera property there was a shot to extract material and that with the wind the dust reached the area EE was working and fell into EE's throat. EE indicates that at that time of the incident was not wearing mask.
The employee indicates that going down the stairs, EE's right leg fails and slips. Fell sitting down and lying without air with a lot of pain in the lower back.

The employee indicated EE was welding the bucket and did not realize that the cable cover was defective and having contact with the bucket created a spark and caused a small burn on right leg. EE was checked, cured and indicated that EE could continue working. EE continued work normally until 10-6-21 which indicates that EE is uncomfortable with the area.
The employee indicates that on Saturday employee was assembling a 335 Excavator road chain with a bar. When employee forced self to join the links, employee felt severe pain in right leg (in the groin). The employee indicates that employee thought it was nothing, but since yesterday employee has been feeling a lot of pain in the right leg (in the groin).
The employee indicates that while performing maintenance work, employee coughed and felt severe pain in waist. The pain makes it impossible for employee to work.
Indicates the employee was uncovering the "muela" (quarry mill) using the sling (eslinga) that adapts to the patella, the sling was stripped. EE tried to unlock it with a rope and a stone fell, hitting EE in the left arm. The employee reported to work the next day and EE's arm was swollen and in pain. The employee was absent on April 7 and goes to the private doctor.
The employee indicates that yesterday afternoon while cleaning the "muela"(quarry mill), they felt severe pain in their back. As they were about to leave, they did not report, but they could not sleep because of the pain. Today they could not start their work.
The employee indicates that while EE was cleaning on conveyor # 9 with a shovel, EE felt severe pain in their back.
The employee reported to work on 2-16-2021, but indicates that EE cannot work due to severe pain in EE's lower back.
Today, January 26, 2021, the employee indicates that when employee finished work schedule on Sat., January 23, 2021, employee cleaned the cab & the ladder of the truck with water. As employee was getting out of the truck, employee slipped & grabbed the handrail. In those moments employee felt pain in left arm & back. The employee did not report to work on Mon., January 26, 2021.

While the employee was pulling a hand truck that was jammed, when it freed itself, it impacted EE on the index finger of the right hand.
Before starting work at 7:30 am, 8/13/20, the Supervisor noticed that the employee was in pain. Employee reports that in the early morning (2:00 am) they felt severe pain in back. EE was advised that they could not work under those conditions and that they could go to the CFSE. EE decided to rest for a day. On 8/14/20, the employee reported back pain and is referred to the CFSE.
The employee went today to a hospital because of a pain in left thigh and the doctor gave the employee rest until August 6. The hospital relates pain to work.
The employee was strapping a stone with a chain. The chain was tied to the bucket of an excavator. When the chain stretched, the index finger of the right hand was caught

The employee was repairing the bucket of the Loader Caterpillar 988G. To perform this work the employee was forcing an iron into the bucket using a hydraulic jack and a hammer screw. The screw detached from the Hydraulic jack and hit it on the head
When the employee was lowering the Excavator to repair the air conditioner, EE hit the tree and there was a bee hive and the bees stung EE all over EE's body.
The employee indicates that while cleaning the belt roller #18 employee felt a strong pain in the back.
While the employee was in the welding shop welding the base of the teeth of the bucket, ee took off the welding mask and started to remove the surplus of the welding with a pickaxe. When ee hit the surplus a hot particle hit ee on ee's left eyelid.

The employee alleges that while he was applying heat and pressure to a bushing (buje) to take out a component, this came expelled from the shaft(eje) and fell on the acetylene hose. The hose was broken and the shooting sparks fell on his left forearm.
The crew dedicated to the primary plant, including EE, was doing adjustments to the primary apron feeder. EE left the area where the rest of the employees were working and went towards the area where the guard of pulley of the feeder, removed at the time for adjustments. EE allegedly tripped on his own feet and hurt his hand on the motor sheave.

EE claims that on December 16, 2009 he started having a numbness sensation on his legs. On December 17th he went to the emergency room to receive treatment. The employee claims that he is still very symptomatic and on December 21st he went to CFSE to receive treatment.
When ee was mounting a transmission of a truck (7738) he felt a strong pain in the back and he could not continue with his work.
Employee was using a digger to clean up, it hit a rock, digger trembled and this affected his neck (muscular pain).

HE WAS PRONOUNCED DEAD ON ARRIVAL.
