The employee was a passenger in a golf cart and while going up hill, the cart lost power and when the brakes were applied, they failed. The employee jumped from the cart and when EE landed on the slanted ground, EE received a minor blow to the head, in which most of the impact was taken by EE's left hip, lower back and receive lacerations to both elbows.

A welder had an excavator bucket held upright by a metal leg. To lay the bucket down, EE attached a chain to it and removed the metal leg. As the chain lifted, the bucket laid itself towards the welder and caught EE between the bucket and a table that had a welding machine on it, injuring EE's left abdomen and left leg.

The employee indicates that while they were doing preventive maintenance (oil and filter change) on the John Deere 350D when they released the engine protector cover it fell on their left hand

The employee indicates that on Saturday, June 24, while EE was cleaning the tools with the pressure machine, EE injured left hand with the pressure of the water.

The employee reports today, june 16, 2023, that on Monday, June 12, 2023, while working on the installation of a water pipe on the Cantera property there was a shot to extract material and that with the wind the dust reached the area EE was working and fell into EE's throat. EE indicates that at that time of the incident was not wearing mask.

The employee indicates that going down the stairs, EE's right leg fails and slips. Fell sitting down and lying without air with a lot of pain in the lower back.

The employee indicated EE was welding the bucket and did not realize that the cable cover was defective and having contact with the bucket created a spark and caused a small burn on right leg. EE was checked, cured and indicated that EE could continue working. EE continued work normally until 10-6-21 which indicates that EE is uncomfortable with the area.

The employee indicates that on Saturday employee was assembling a 335 Excavator road chain with a bar. When employee forced self to join the links, employee felt severe pain in right leg (in the groin). The employee indicates that employee thought it was nothing, but since yesterday employee has been feeling a lot of pain in the right leg (in the groin).

The employee indicates that while performing maintenance work, employee coughed and felt severe pain in waist. The pain makes it impossible for employee to work.

Indicates the employee was uncovering the "muela" (quarry mill) using the sling (eslinga) that adapts to the patella, the sling was stripped. EE tried to unlock it with a rope and a stone fell, hitting EE in the left arm. The employee reported to work the next day and EE's arm was swollen and in pain. The employee was absent on April 7 and goes to the private doctor.

The employee indicates that yesterday afternoon while cleaning the "muela"(quarry mill), they felt severe pain in their back. As they were about to leave, they did not report, but they could not sleep because of the pain. Today they could not start their work.

The employee indicates that while EE was cleaning on conveyor # 9 with a shovel, EE felt severe pain in their back.

The employee reported to work on 2-16-2021, but indicates that EE cannot work due to severe pain in EE's lower back.

Today, January 26, 2021, the employee indicates that when employee finished work schedule on Sat., January 23, 2021, employee cleaned the cab & the ladder of the truck with water. As employee was getting out of the truck, employee slipped & grabbed the handrail. In those moments employee felt pain in left arm & back. The employee did not report to work on Mon., January 26, 2021.

While the employee was pulling a hand truck that was jammed, when it freed itself, it impacted EE on the index finger of the right hand.

Before starting work at 7:30 am, 8/13/20, the Supervisor noticed that the employee was in pain. Employee reports that in the early morning (2:00 am) they felt severe pain in back. EE was advised that they could not work under those conditions and that they could go to the CFSE. EE decided to rest for a day. On 8/14/20, the employee reported back pain and is referred to the CFSE.

The employee went today to a hospital because of a pain in left thigh and the doctor gave the employee rest until August 6. The hospital relates pain to work.

The employee was strapping a stone with a chain. The chain was tied to the bucket of an excavator. When the chain stretched, the index finger of the right hand was caught

The employee was repairing the bucket of the Loader Caterpillar 988G. To perform this work the employee was forcing an iron into the bucket using a hydraulic jack and a hammer screw. The screw detached from the Hydraulic jack and hit it on the head

When the employee was lowering the Excavator to repair the air conditioner, EE hit the tree and there was a bee hive and the bees stung EE all over EE's body.

The employee indicates that while cleaning the belt roller #18 employee felt a strong pain in the back.

While the employee was in the welding shop welding the base of the teeth of the bucket, ee took off the welding mask and started to remove the surplus of the welding with a pickaxe. When ee hit the surplus a hot particle hit ee on ee's left eyelid.

The employee alleges that while he was applying heat and pressure to a bushing (buje) to take out a component, this came expelled from the shaft(eje) and fell on the acetylene hose. The hose was broken and the shooting sparks fell on his left forearm.

The crew dedicated to the primary plant, including EE, was doing adjustments to the primary apron feeder. EE left the area where the rest of the employees were working and went towards the area where the guard of pulley of the feeder, removed at the time for adjustments. EE allegedly tripped on his own feet and hurt his hand on the motor sheave.

While the mechanic assistant was helping to set the points of the transmission in the Loader 988, when the mechanic assistant was helping him to putting the last points of the transmission the jack slipped away and felt the transmission to the tool that was being used as chock and impact him on the index finger of his left hand.

While the employee was repairing the piston of a steering bottle from the 773B when loading it he felt a strong pain in the neck and the back.

The EE says "When I was riding on the machine slip and I hit my left ankle."

EE claims that on December 16, 2009 he started having a numbness sensation on his legs. On December 17th he went to the emergency room to receive treatment. The employee claims that he is still very symptomatic and on December 21st he went to CFSE to receive treatment.

When ee was mounting a transmission of a truck (7738) he felt a strong pain in the back and he could not continue with his work.

Employee was using a digger to clean up, it hit a rock, digger trembled and this affected his neck (muscular pain).

The EE heard a noise and he went down to check the machine, in the distance there was a hydraulic hammer, a stone came out and impacted his forehead.

While loading the oil drum, it slipped and when he tried to stop it, the drum caught his left ring finger against the truck.

It was a very rainy day. The employee was driving towards millstone with a loaded 773B. When he got to the curve it slipped on the "verma". His right leg received an injury.

EE was changing the chain on the D9R dozer. When he hit the hammer to the shoe, a fly passed through the security spectacles and impacted his left eye.

While removing a radiator from an equipment, he lifted it (to pull it out) to position it closer to the door of the machine and it slipped falling on his pinky finger (right hand) causing it to break (tear) the skin and remove the fingernail.

THE MINER WAS WASHING HIS TRUCK WHILE STANDING ON THE BALCONY. HE SLIPPED AND FELL TO THE GROUND INJURING HIS LEFT ELBOW

OPERATOR WAS WORKING ON FACE OF OPEN PIT WHEN REPORTEDLY BOULDER HIT SIDE OF THE MACHINE. OPERATOR REPORTED BACK PAIN AFTER IMPACT. OPERATOR SUFFERED FROM BACK PAINS BEFORE ACCIDENT. THE MACHI NE SUFFERED ONLY DOOR DAMAGE.

WHEN EE WAS PULLING UP A CRYSTAL OF THE MACHINE HE FELT A SMALL PARTICLE IN HIS LEFT EYE.

WHEN EE WAS WORKING ON THE PLANT THE EE SLIPPED AND FELL ON HIS RT. HAND INJURING HIS LITTLE FINGER (METIQUE).

WHEN EE WAS SHARPENING A SAW TO WORK ON A TASK HE ACQUIRED A CUT ON AN INDEX FINGER ON HIS RT. HAND.

WHEN EMPLOYEE WAS CLEANING THE MILL HE GOT UP A METAL COVER AND HE FELT A PAIN IN HIS BACK.

WHEN EMPLOYEE WAS GOING DOWN OFF THE MACHINE FOR COVER A HE FELT PAIN IN HIS BACK.

WHEN EE LOADING A DRILL HE FELT A PAIN IN HIS BACK.

WHEN EE WAS HELPING TO CHANGE SCREENS OF THE VIBRATOR WHEN ANOTHER EE WAS REMOVING A SCREW WITH A TORQUE WRENCH PUNCTURE HIS FINGER

WHEN EE WAS CLEANING HIS WORK AREA HE FALL AND THE "TRIMER" HIS HIM IN HIS RIGHT HAND.

WHEN EE WAS CLEANING THE CONVEYOR. HE WAS IN THE PROCESS OF GET OUT THE MATERIAL LAND HIT HIS RIGHT HAND WITH AN ANGULAR.

WHEN EE WAS CLEANING WITH THE MACHETE, IT FALL CUTTING HIM IN HIS LEFT HAND. MIDDLE AND RING FINGER.

WHEN EE FINISHED THE BLASTING, HE SLIPPED & FELT A STRONG PAIN IN HIS LEFT KNEE.

THE PERSON AFFECTED WAS ON HIS WAY TO THE EXTRACTION AREA WHEN HE SUFFERED AN APPARENT HEART ATTACK. THEN 2 OF HIS CO WORKERS LAYED HIM FLAT ON THE BED OF A TRUCK. THEN THE OFFICE SECRETARY RE QUESTED AN AMBULANCE VIA 911. AFTER NOTICING THAT, HIS SITUATION KEPT GETTING WORSE, THE PLANT MANAGER DECIDED TO TAKE HIM PERSONALLY TO THE HOSPITAL. HE WAS PRONOUNCED DEAD ON ARRIVAL.

WHEN EE WAS WOKING IN THE CATERPILLOR D8H, THE MACHINE BROKE AN HYDRAULIC SLEEVE, THE OIL FELT INTO A MANIFOLD PRODUCING FIRE & THE FAN MOVED THE FIRE TO THE EE. HE WAS BURNED IN HIS RIGHT HAN D, THE NECK & PART OF THE FACE.

WHEN EMPLOYEE WAS CLEANGNN THE SOLDERING SHOP, A PIECE OF A CONVEYOR (ROLO), FALL INTO CHG INDEX FINGER OF HIS LEFT HAND.

WHEN EMPLOYEE WAS CLEANING A TRUCK MODEL 773 B WITH A PRESSURE HOSE, A PARTICLE FELL INTO HIS LEFT EYE.

WHEN EE WAS HELPING OTHER TO PICK UP A DRILL HE FELT A PAIN IN THE RIGHT SIDE OF HIS BACK.

THE EMPLOYEE WAS CLEANING THE FUNNEL IN THE PRIMARY WHEN HE PICKED UP THE PICKAXE INJURED HIS LEFT HAND THUMB WITH THE METAL.

WHEN EMPLOYEE WAS WALKING DOWN WITH A DIESEL PAIL HE SLIPPED. AT THE MOMENT HE DID NOT FEEL ANY PAIN, BUT THE NEXT DAY IN THE MORNING HE BEARLY COULD GET UPO THE BED BECAUSE FELT A LOT OF PAI N ON THE RIGHT SIDE OF HIS WAIST.

WHEN EMPLOYEE WAS CLEANING UNDER THE CONVEYOR #3 HE TURNED WITH THE LOADED SHOVEL AND HE FELT PAIN IN HIS WAIST.

WHEN EMPLOYEE WAS WALKING DOWN THE STAIRS,HE SLIPPED AND TRYING TO HOLD ONTO THE HANDRAIL HE HIT HIS KNEE WITH THE WALL.

EMPL WAS COMING FROM PLANT IN CHEVROLET PICKUP IN DIRECTION TO CLOCK WHEN HE GO DOWN FOOT SNARE & HE FALL INJ IN BACK PART.

WHEN EMPLOYEE WAS ASSEMBLING A TIRE TO A TRUCK THE CHAIN (THE ONE THAT WAS USED FOR ASSEMBLY THE TIRES OF THE TRUCK) CUT THE TOP OF HIS RIGHT LITTLE FINGER.

EMPLOYEE WAS SOLDERING THE THUMB OF THE TRUCK WHEN HE GOT DOWN HE SLIPPED FALLING IN THE GROUND.
