D and S AgStar · Caught in, under or between running or meshing objects
Employees were dislodging 3" rock from tail pulley on conveyor. Co-worker engaged lever for conveyor to start as EE reached into conveyor causing his left arm to become stuck for approx. 30 to 40 minutes. EE was transported to the hospital where he received stitches to re-attach bicep muscle. All employees have been refreshed on lock out tag out.
