WHILE REMOVING FROZEN MATERIAL FROM DRY PLANT CONVEYOR A FROZEN PIECE BROKE OFF WHILE I WAS PUSHING ON IT. MY HAND ROLLED ONTO ITS SIDE BREAKING MY SMALL FINGER ON MY LEFT HAND.

WHILE REPLACING A BELT EE SAFETY GLASSES BROKE. HE INFORMED ME IMEDIATELY. WHILE I WENT FOR ANOTHER PAIR EE ATTEMPTED TO LEVEL THE WORK AREA. HE STRUCK SOME FROZEN AGGRYRATE WITH A HAMMER CAUS ING SOME DIRT TO FLY UP AND HIT EE IN THE FACE AND LEFT EYE. WE TRIED TO FLUSH HIS EYE UNSUCCESSFULY. WE TOOK HIM TO THE PAC CENTER WHERE THEY PUT A PATCH OVER HIS SCRATCHED EYE AND PRESCRIBE
