Periama Handling, LLC · Struck by falling object

A small lump of coal fell from fixed belt (portable coal crusher) and struck the ee startling him. He reacted by twisting away from the machine. He felt a pain in his lower back after doing this. He chose not to seek medical treatment, working in a light duty rok through 4/8/13. On Sat 4/6/13 - he wake up in pain and sought medical attention.

While attempting to service (greasing) a rubber tired end loader he was stepped up on the boom where the bucket attaches and was reaching to grease a fitting toward middle of machine. He lost his footing and fell toward bucket bruising his left hip and buttocks and strained his left leg.

THE DAY BEFORE THE ACCIDENT THERE HAD BEEN A RAMP ON THE END OF THE STOCKPILE THAT HE DROVE OFF OF. ACCORDING TO HIM, HE ASSUMED THAT THE RAMP WAS STILL
