EE did not follow safety procedures of lockout and tagout, and proceeded to adjust door on hopper while conveyor belts were in motion.

Employee was standing on conveyor of the powerscreen, putting tools in bucket of Bobcat. Employee slipped and fell off of the powerscreen, about 3 ft in height. Employee did have safety harness on at the time causing employee to be pulled back towards the Bobcat bucket. Employee's legs hit the Bobcat bucket causing a laceration to left lower leg.

Miner was standing on second deck of screens inside a 3 deck screen with top screens removed and his foot was against a rail, as he turned his upper body his foot stayed stationary against the rail and he twisted his knee, causing some soreness.
