Employee was helping tighten bolts on a cone crusher. He was using a 2 lb hammer and a wrench. While striking the wrench with the hammer, he missed the wrench and hit his 3rd finger on his left hand, breaking his finger.

Employee was removing rock from crusher and pulled muscle in his lower back.

EMPLOYEE WAS WORKING AS LABORER AROUND CRUSHING CIRCUIT. WHEN HE WAS FINISHED, HE STEPPED OFF OF CONCRETE PAD ONTO A ROCK HE DID NOT SEE AND TWISTED HIS RIGHT ANKLE.
EE WAS ATTEMPTING TO REMOVE A LODGED JAW PLATE AFTER REMOVING HOLDING PINS AFTER JAW COULD NOT BE REMOVED. CRIBBING HAD BEEN USED TO REMOVE PINS AND WAS REMOVED TO EXTRICATE JAW PLATE. WHEN JAW PLATE COULD NOT BE REMOVED, CRIBBING WAS NOT USED WHEN ATTEMPTING TO FREE JAW PLATE AND PLATE SHIFTED, CAUSING EE'S RIGHT FOOT TO BE TWISTED.
EE WAS STEPPING OUT OF THE CAB OF THE FORD BOOM TRUCK WHEN SLIPPED AND FELL ON SPILLED DIESEL FUEL. EE FELL APPROX 45" TO THE GROUND. EE HAD FUELED THE BOOM TRUCK AND SPILLED THE FUEL.

OPERATOR WAS ON D25C CAT HAUL TRUCK WHEN DASH OF HIS TRUCK APPARENTLY SHORTED OUT & CAUSED A FIRE, SMOKE FROM THIS FIRE WAS INHALED BEFORE THE TRUCK COULD BE STOPPED & THE DRIVER COULD GET OUT OF THIS CAB.
WHILE REPLACING BELT DRIVE ON MOTOR TO FEED BELT TO SCALPING SCREEN, FINGER CAUGHT IN PULLEY WHILE CHANGING BELT. RESULTED IN A FRACTURE TO THE AREA BETWEEN THE KNUCKLE AND 1ST JOINT. SUTURES WERE USED ON A SMALL LACERATION IN THE AREA BETWEEN THE 1ST JOINT AND FINGER TIP.
