EMPLOYE WAS CRAWLING ALONG ENTRY AND PIECE OF ROCK FELL OFF ROOF BOLT AND HIT RIGHT LEG AND BROKE BONE IN LEG ABOVE ANKLE.
ROOF BOLTER BOUNCED OPR INTO TOP HURTING NECK.
EMPLOYEE TOOK SICK UNDERGROUND AT DINNER HOLE AND COMPLAINED TO ANOTHER EMPLOYEE THAST HIS HEAD WAS KILLING HIM. HE WAS BROUGHT TO THE OUTSIDE AND SENT TO HOSPTIAL BY AMBULANCE THE DR EXAMINED HIM AND FOUND HE HAD A VEIN BURST IN BACK OF HEAD THIS REPORT IS FOR A RECORD OF INCIDENT ONLY.
EMPL WAS SLIDING ROCKDUSTER INTO BUCKET OF SCOOP AND HURT HIS BACK.
