Two maintenance mechanics entrapped in the Mill Bldg. elevator after the car stalled between the 2nd & 3rd floor landings. Both employees are uninjured and remain in constant communication with site personnel. Otis Elevator has been notified, a technician was dispatched to the site to safely extract the employees and evaluate the equipment. MSHA was notified immediately.

While operating a John Deere 315 Skid Steer to transfer cement powder out of the mill building as part of a cleaning task, an operator wearing steel-toed boots rested their feet outside the footwell with the bucket slightly raised. This resulted in injuries to the right big toe.

Two production assistants were trapped in an elevator at the cement silos. Plant personnel could not get the car to move. Otis Elevator was called for assistance. After opening the top trap door for a visual, the uninjured workers were able to exit safely using a 12-foot ladder lowered from the floor above. MSHA was immediately notified of the incident.
Cutting Flange Bolts on Shroud from Jib Crane. Shroud fell on Steel toed boot and bounced up onto metatarsals of right foot fracturing #2 & #3 bones

While moving pallets a fork came off the forklift. While trying to put the fork back on the EE got their ring finger (right hand) pinched between the fork and the mast.
Employee was moving a drum and the drum cut the left thumb.

The control room received a stall alert on conveyor belt #60. The control room supervisor inspected the cause and observed that there was a fire in the tube that leads from the transfer tower to the clinker dome. Fire Department was contacted immediately.

The overload was tripped causing the elevator car to stop. The employee did NOT experience any Health or Safety issues while trapped in the elevator.

Employee was using the Shear machine to trim metal when the right index finger was pinched.
Employee operating drill motor with EE's right hand when EE's left hand came into contact with the rotating roll pin. The rotating roll pin twisted into the employee's glove and resulted in a laceration in the webbing between EE's left thumb and left index finger. Employee received stitches, and returned to work on modified duty.

Delivery driver attempted to descend down stairs of the loading ramp when the driver slipped and fell down the rest of the stairs, landing on the ground.

Employee LOTOTO K1 coal mill rotary feeder. Employee unclutched hand wheel and tried to spin it to free up blockage. Employee received laceration to left hand, between the thumb and index finger. Employee was not wearing gloves. Hand wheel looked to have a rough edge on the inside from casting. Employee received 3 stitches and tetanus shot, then returned to work on light duty.
IMG employs were assembling buckets to waste elev. chain. Employee went to straighten a piece of chain and finger was caught between 2 buckets. This caused a laceration to the right index finger. The Risk Assessment and JHSA both advised about pinch points. IMG employees signed off at the safety meeting that morning of the risks related to this job. Employee was wearing gloves.
EE was cleaning files out of filing cabinet. Placing the files in garbage bag to be shredded. Once file cabinet was cleaned out, EE went to lift the garbage bag and felt a severe burning pain down the left arm to where pinky and ring finger had burning sensation. *6/15/17 EE was seen at the Ortho Institute and given 2 injections/2 prescribed meds/and referred for MRI & PT*

EE was walking on a concrete paved area, when the EE slipped on a small, thin patch of wet raw mix. His left foot came to a stop on dry concrete causing his left knee to overextend. EE reported as a precaution. EE reported discomfort recurred and worsened after a couple of months. EE went to the doctor for evaluation, and returned to work the same day on modified duty.
Cutting a water pipe with gas powered cut off saw. Kickback from the saw caused the blade to lacerate his chest.
While unthawing water lines, pressure accumulated in lines; pressure was initially released. Pressure accumulated again & while releasing safety wire from clips holding pipe fittings together, hose broke loose striking employee in the nose and under right eye.

Ee was climbing a fixed ladder to access a light fixture for repairs when he hit his head on a pulley platform. Ee complained of neck pain and applied ice to the area. Symptoms improved and RTW with no restrictions. On Jan. 22, 2015, symptoms returned, ee was seen by a medical physician who prescribed physical therapy and prescription with no restrictions.
EE was inspecting belt. EE safety glasses were sprinkled with dust. EE removed glasses and washed face. Next day ee's left eye was irritated. Medical Professional examined both eyes and reported a small abrasion on left eye. EE was prescribed an antibiotic and was released to full duty with no restrictions the same day.
Ee was using air lance. Rt eye felt irritated after task was completed. Began eye irrigation with supervisor. Small particle from his upper eyelid was removed. The EE was then taken to a medical provider. Both eyes were examined and found an abrasion on the right eye. EE was prescribed an antibiotic and was release to full duty with no restrictions the same day.
Employee strained his lower back while swinging a sledge hammer in a crouched position, when he missed his target impact point.

EE pulled his hard hat off to insert his welding hood. At that time dust fell from his hard hat & behind his safety glasses, into his eyes, causing a burning sensation eyes were flushed on-site, then taken to a medical professional where he received two prescriptions for his eye injury(ointment& eye drops).RTW the next day without restrictions.
EE rec'd laceration to finger. Treatment included washed/bandaged/splinted/sutures. EE was loading a barrel onto a flat-deck barge, and when sitting the barrel on the barge his right ring finger was caught between the barrel and the barge deck causing a laceration. EE was wearing gloves while performing task. He returned to work with no restrictions.
Ee called in to check damage of electrical circuit due to fire. Ee assessed and returned home. EE returned next am for normal shift and felt lightheadedness/chest discomfort. Ee was treated by medical provider for Smoke Inhalation and returned to work same day to regular activity with no restrictions.

On July 17th Employee replaced four dust collector bags and on July 20th was diagnosed and treated for dermatitis in the neck area. No lost time or modified duty.
Mechanic hung tool belt on equipment stored on shelf while placing tool in belt, equipment fell striking employee.

Mechanic suffered a laceration to his Left thumb, on the dorsal side, while drilling a hole in plumbers strap to secure the air line seal air on R1 pump discharge bearing. The drill caught and the metal curled toward him and cut his Left thumb between the 2 joints.
EE was picking up a barrell of graphite to put into the bed of a mule. When he lifted he felt a hot pain in groin area.
EE Was reaching for vacuum hose when his left arm was caught in hose resulting in swelling and abrasions.
QC Tech was traveling on FM2 grated platform to obtain a grinding aid sample. Uneven grating caused him to fall. He landed with his weight on his right arm and right knee. Currently, his knee is just scraped, but his elbow and forearm are in alot of pain
EE was using an air lance 2 dislodge remaining mat'l with air. He jabbed at the blockage 2x & hot mat'l came out door burning ee ear. He then came 2 ctrl room 4 assistance. At no time did we believe that there was any hot mat'l remaining to drop above the tipping valve door as it was completely cleaned out during 1st step of job. Additional details available, no room 2 input.
While employee was raising one side of a semi tire from a horizontal position to an upright position, the employee felt a sharp pain between his shoulder blades.
2 mechs. were at the head pulley shaft guard on 12 belt getting measurements to make new guards.The mech. had his hand near the top of the guard. He placed his hand on the guard causing it 2 come in contact with the bolt at the hd pulley taperlock bushing.The bolt caused the guard to jump & pinch the mech hand.The cut is at the base of his left ring finger,9 stitches were required.

During plant clean up day EE was shoveling dust and noticed that his right elbow was hurting. EE did not think much about the pain because this is a task he doesn't normally perform in his regular job duties. EE went to Occunet on January 31, 2011. EE had e-rays and received medication at that time.
Cement dust in left eye. Employee states that he felt some cement dust in his left eye and went to irrigate it out. He did not notice a foreign body at all, only irritation. Employee states he was wearing his safety glasses. Dust caused abrasion to soft tissue. Prescription meds were prescribed to heal.
Employee was one of four workers sliding #11 rebar on top of channel iron installing rebar mat. Employee got right hand caught between iron and rebar causing abrasion.
Employee worked for 2 days, during peak summer, on jobs that were in high heat areas (mill building, mill building & storage crane)where the temperatures were around 110 degree F & very little air movement. He spent most of the day working on Raw Mill Air Separators in the mill building. He did feel hot, but did not see any warning signs of heat exhaustion.
Hydraulic wrench slipped and hit the right lateral side of his calf and caused pain to the right medial side of his knee. EE was examined and returned to full duty, however, in a follow up exam and MRI shown there was a torn ACL, MCL and mincus. He was put on modified duty on August 2, 2010.
EE was cleaning concrete. He turned to place a piece on a platform and while lowering the chunk, he mashed the tip of his right middle finger between the chuck and the ground. His finger tip burst and he went to the clinic for treatment. The doctor bandaged the finger and will re-evaluate his finger in 2 weeks.
EE was trying to unlodge an air lance. The air lance came loose and the EE hit his elbow against a handrail. The EE went to the doctor on 6/18/2010 for precautionary measures and was placed on modified duty and due for a re-evaluation on 6/28/10.
Ee was lifting a large water bottle into a vehicle and his elbow started to hurt afterwards. The Ee tried to treat the discomfort with rest and OTC medication. After no relief, the Ee went to the doctor and was placed on modified duty for two weeks with re-evaluation on 6/28/10. (Original injury date was 3/30/10 and when he finally went to the doctor on 6/18/10)
E-TECH was troubleshooting electrical problem with belt conveyor. Working in Major Control Center, motor starter bucket. Bending down on knees for work, went to stand up and left knee popped. Knee is real stiff now & sore!
Employee was walking to break area, stepped on a piece of material which rolled his ankle causing a sprained ankle.
Employee was in the storeroom cutting a hose and cut his left thumb.
Changing forks on fork lift at work fell with hand under it injury to left 4th digit.

Employee was trying to remove a tire stuck in the tire elevator on the kiln TDF system, when the tire released and struck his left forearm.
While EE was emptying the 2 yard tire overflow bin, The EE reached horizontally into the bin to pull a car tire (less than 25 lbs) out and felt a sharp pain between shoulder blades.
Working under structure, slipped and placed all weight on right knee. Caused knee pain.

Annual Hearing Test showed Recordable Shift in Hearing for employee.
Mechanic got clinker dust in right eye when he was crawling into undergate compartment on the cooler.
Employee was collecting samples of cement; cement in jar popped out and blew all over, getting into his left eye.
Employee squatted down to release a pipe and heard left knee pop.
Employee was bending a valve handle over and it broke. A PC approx 8" long struck his left knee.
EE was removing a wear sleeve from a pump screw, using a hammer and chisel. He missed the chisel and struck his left hand in the area of the thumb.
EE was on top of the Finish Silo's for a ground level fall injured while measuring E Pocket canister. He slipped from a cement line that he was standing on to measure the pocket. Twisted right ankle.
Employee was inspecting work in kiln. As he walked he stepped onto bare kiln shell and slipped, landing on his shoulder.
2x4's were laying on overhead plastic and were not seen when removing plastic.

Mechanic was opening new boxes of dust collector cages, as he was pulling the cardboard box out from under the cages the back of his hand struck the top band of the cage and cut his hand.
Temporary Agency employee, reported that he had squatted down on one knee to lift a tire to its side so the tire could be rolled into position for disposal. In doing this, ee stated that he pulled a groin muscle. Employee reported his injury and then went home due to his pain. Later employee followed the Temp Agency policy and reported to their doctor for medical inspection.
WORKING ON SETTING MOTOR ON F2 HES SEPERATOR, EMPLOYEE TWISTED HIP AND POPPED HIP JOINT. EMPLOYEE WENT TO THE DOCTOR AND RECEIVED MRI ON JULY 19. WE'RE AWAITING RESULTS FROM MRI AND THE DOCTOR. THE EMPLOYEE CONTINUED TO WORK WITH NO MISS TIME. HE IS EXPERIENCING SOME TINGLING PAIN.
Employee was shuffling ball mill liners inside the mill when a liner shifted and smashed the tip of his pinky finger.

While assisting other co-workers with changing silos,the co-workers were using tools to releive the valve when a piece of metal flew off and went into the employees right forearm causing a laceration to his right forearm. Although this occured 8/28/06 there was no medical treatment other than X-Ray until 9/14/06, when metal was removed from right forearm.
While decending #34 conveyor belt walkway, employee slipped on debris/material builtup on walkway. Later, EE decided to have the knee checked due to swelling. EE visited and received an MRI on 4/17/06. As a result surgery was scheduled and performed on 4/20/06. EE is currently recouperating at home after a surgery to repair a tear.
Employee was shoveling in rail pit and felt something pull in the groin area. Employee was just recently diagnosed with a hernia condition and had surgery on March 6, 2006. He is expected to return to work on March 20, 2006.
Shift in hearing baseline.
While working on the 2nd step of a 6' step ladder, I began climbing down the ladder and missed the 1st step of the ladder and fell to the floor. When I landed on the floor, I struck the floor with my left elbow, causing me to jamb my left shoulder.Although this accident occured 1/13/06, there was no reportable medical treatment until 9/14/06.

The Employee was using a hand grinder on a conveyor belt when a wheel came off the conveyor belt hitting him and causing a laceration to the left middle finger.
Removing chunks from behind the #3 LSI feeder weigh belt material depth gate. The gate has to be pryed up to remove chunks. Employee was prying up the slide gate and slipped off the lip of the gate causing his hand to drive into the bin bottom slide cut off gate smashing and fracturing his right little finger tip.
Employee was operating a loader. The right side window was open and employee was backing up and the right window slammed shut, breaking the glass. He then parked the loader noticing that he received a laceration to the right arm from the glass.
While cleaning rock from barge that the crane had missed or could not get. The employee was operating the skid steer loader when it caught on a weld in the barge and jolted the employee sideway causing employee to hit the right side of head on the R.O.P.S. cutting employee's right ear.
The employee was climbing a ladder to install a hose in an oil tote when he lost his balance and fell backward. The employee experienced swelling in his left wrist and hand/arm as a result. On 8/15/05 additional testing determined that there were 2 small fractures and they put his wrist and hand in cast. Report was delayed due to final diagnosis.
Employee was starting a 2" gasoline powered water pump, when pump "kicked back" causing the pull cord to jerk, pulling the employee's right hand. Employee's hand sruck the pump causing a laceration between the index and middle finger on right hand, requiring medical attention, and resulting in 10 stitches.

Employee bumped head while walking in low headroom area. Employee stooped down to go under an "I-Beam" , raised up and hit head on 2nd "I-Beam". Stiff -sore neck resulted. Employee went to physician 7/29/04.
Employee was installing V-Belts, on a screw conveyor, when screw conveyor started and tried to run, pulling employees left index and center fingers between the V-Belts and the drive sheave. Employee was taken to hospital and found he had both fingers broken, lacerations, and missing finger nails from both fingers. Employee was treated and released from hospital.
EE FELL FROM LADDER, LANDING ON RIGHT SIDE ARM, RIBS, HAND AND TAILBONE.

WHILE INSTALLING MILL SCREEN PLATES, EE WAS STRUCK IN THE FACE/MOUTH WITH SLEDGE HAMMER.

OTHER DID NOT REALIZE WHAT HE HAS A SKED-HE GRABBED THE BOTTOM OF THE HAMMER BLIND AT THE SAME TIME OTHER PULLED THE TOP. HE WAS ASSIGNED TO CLEAN #11 BELT AND AS HE WAS WALKING DOWN #11 BELT STEPS HE LOST HIS FOOTING ON CLINKER BALLS LAYING ON STEPS. HE LOST HIS FOOTING RESULTING HIM TO FALL BACKWARDS LANDING O N STEPS AND HE TOLD ME HIS LOWER BACK WAS HURTING.
HE WAS BLOWING DUST OFF BEAMS AND HE SHUT OFF AIR AND HE LAYED HOSE DOWN AND HE GOT HOSE HOOKED ON LEG AND TURN AIR ON CAUSING NOZZLE ON END OF AIR HOSE TO FLY UP AND HIT UPPER LIP CAUSING CUT
HE WAS WATCHING BALLS COME OUT OF BALL SORTER INTO 55 GALLON BARRELS. WHEN A BALL MISSED BARREL, HE REACHED TO PICK IT UP AND TURNED AND ANOTHER BALL MISSED STRIKING AND BREAKING HIS FRONT TOO TH.
HE WAS WORKING ON BALL SORTER. HE WAS OUT HOPPER ON BALL SORTER WHEN HE WAS GETTING OUT OF HOPPER, HE STEPPED DOWN AND LOST FOOTING CAUSING HIM TO FALL BACKWARDS ON A OFF-THREADED BOLT. THE BO LT STRUCK HIM BEHIND HIS TESTICLES BRUISING HIM.
