THE EMPLOYEE WAS ATTEMPTING TO CLEAN OUT A CHUTE ABOVE THE HAMMER MILL. HE CLIMBED UP ON THE CONVEYOR BELT THAT WAS WET & HAD SOME MATERIAL ON IT, HE THEN LOST HIS FOOTING & FELL APPROX. 8'.
