Employee was repairing a Raymond Roller Mill Journal assembly. EE was lifting the part with a sling and forklift to position it on a work table. EE attempted to reposition the part before placing on the table when the assembly came loose and a portion fell crushing EE's right thumb.
Employee was carrying a printer down a set of stairs when EE slipped and fell.
Employee was mounting a CAT262D skid loader. There was damage to the foot hold causing a gap. The employee's foot went through the gap as they were twisting to sit in the seat causing a sprain to the left ankle.
Employee was loosening a bold on a Derrick Screen system during a screen change. The nut gave way while EE was using a wrench and the employee felt a "pop" in EE's shoulder.
Employee was cleaning the floor with a broom when EE's hand contacted a cabinet that had a sharp object sticking out. EE had a small laceration on EE's right pointer finger knuckle that required stitches.

Employee was bagging 25lb bags at the Rotex #1 bagger system. They experienced lower back pain and was diagnosed with a lower back strain.
Employee was troubleshooting an issue with a California Pellet Mill. Two employees were rotating the pellet die when the glove of one individual became caught between the rolls and the die pulling the employees left middle fingertip into a pinch point causing a laceration and fracture of the fingertip.
IE was cutting open Supersack of material, IE rested supersack on guard & cut open bag w/ knife, plastic liner inside SS slid out & pinched IE's right hand between liner and filter grate.
IE was filling bags at bagging station and moving to conveyor. The following morning EE reported soreness in EE's right shoulder. Sent IE to clinic and diagnosed with right trapezius muscle strain.

IE was walking across yard outside Warehouse #1 and slipped on ice falling to ground. IE fractured left ring finger and bruised left shoulder and knee.
IE was attempting to unclog finish bin by striking with hammer. After several hits, hammer glanced off bin and IE's hand holding hammer struck metal support frame causing a bruise to right hand. IE was taken to health clinic and x-rays were negative for broken bones. IE was given lifting and grasping restrictions requiring IE to be placed on light duty job.
Employee was grinding a metal bracket with a Milwaukee 2785-20 angle grinder and a cutoff wheel. EE was wearing eye protection. EE awoke in the morning of 4/16 with an irritated eye and it was determined EE had a small piece of metal in EE's eye. EE sought medical attention to remove the piece and follow up care including antibiotics.

Employee attempted to step over a bench in the shower room striking EE's foot on the wooden bench causing a fracture to a toe on EE's right foot.
Employee exited the forklift from the right side where the controls are located. EE struck knee on the dash creating a small laceration on the left knee.
Employee experienced a medical emergency that was called in to MSHA at the time of occurrence. There was no injury to the individual, the employee experienced an event related to low blood sugar. They were transported by ambulance to the hospital where they observed them and diagnosed this condition.
Employee reported right shoulder pain while breaking concrete with a sledgehammer. EE documented that EE had a previous injury to the shoulder. EE did not seek medical treatment until 5/11/23 at which time EE reported treatment beyond normal first aid. Diagnosis is "joint pain". No lost time, no restrictions to date, treatment is ongoing.

This is a reportable audiometric summary incident. The date and times are not accurate as this was a result of our annual hearing tests. Employee experienced a reportable change in the right ear.
EMPLOYEE WAS REMOVING ICE FROM A CRUSHER DISCHARGE CHUTE USING A DIGGING BAR WHEN EE'S THUMB STRUCK A METAL BEAM.
Employee slipped and fell in employee parking lot. Monday snowstorm had left ice accumulations that were treated and continued to be addressed as temperatures were fluctuating. Parking lot was plowed and treated with salt on Monday and retreated as the week went on. Employee has strained back and return to work date on 1/24/22.

Employee tripped on the way from the employee parking lot to the plant. It was dark, but the area was illuminated. A small piece of pallet board was in the area and the employee was walking on the stone surface rather than the paved area. EE initially received no treatment, but late last week was then seen by ortho and had treatment and was sent for therapy.
Employee struck finger on angle iron while clearing a jam in the AC Mill system. Employee sustained a sprained finger on left middle finger.
Employee exited company truck to unlock entry gate and twisted knee.
Employee was packing bags and placing on pallets when EE tripped on plastic wrap causing injury to EE's knee. Employee had prior knee surgery and was having issues prior to this incident.

Employee slipped on concrete step. Sore arm, hyper extension right knee. Refused treatment. The employee attended a scheduled Dr appt 8/13/20 for pre-existing condition of right knee. The employer was notified of additional restriction on 8/17/20. This accident sent 8/17/20 and failed to submit documentation of the attempted submission exists.
Employee was dismounting a forklift when EE's foot slipped. Employee landed on left leg and experienced pain in the left knee. Employee was sent to hospital for evaluation and is to return to full duty on 2/17/20.

Employee was changing a screen associated with a small hammermill when EE felt a pull in groin. EE did not report it nor seek medical attention for the injury. On 8/27/19 EE reported EE was diagnosed with a hernia which EE feels was associated with the pull EE felt during the screen change.
Employee was helping to move a band saw when it tilted. The employee fell in attempt to get out of the way, landing on EE's left elbow and shoulder. EE did not seek treatment at the time. EE sought treatment on 7/10/19 and was sent for therapy. Company official who reports was notified 7/31/19.

Employee was descending steps from a loader and slipped. The employee caught self using the hand rail, but strained back.
Employee had an audiometric shift that was reportable.
Employee had purchased new safety shoes. EE was descending a set of concrete stairs and slipped but did not fall. EE had a small scrape on forearm. It was not reported as an accident at the time until the employee began complaining about back pain. The employee stated ee did not know if the slip had caused the issue.
Employee was unlocking and opening the gate to the truck scale. Employee lost hold on the gate and tried to grab it so it would not swing out toward the roadway. In employee's attempt to grab the gate employee overturned ankle causing a sprain.
Employee was treating prior to the event for pain in EE's knee that was not work related. EE was walking down a set of stair when EE reported a "pop" in EE's knee. EE did not fall, EE did not slip and EE did not trip. The employee was wearing a knee brace at the time and had no restrictions from EE's physician.

Employee was cleaning finish elevator #2 when ee was overcome with carbon monoxide.
Employee was removing material from an elevator and sustained a muscle strain in lower neck/upper back area.
Employee was emptying drums of trash. The employee attempted to lift a drum that was too heavy for one person. EE felt a pull in EE's back but refused any treatment. Over the weekend on 8/26-8/27, EE began to have pain in EE's back and was treated on 8/27/17. No diagnosis available as of 8/30/17.
EMPLOYEE WAS STACKING BAGS ON PALLET. EE PLACED A BAG, TURNED TO RETURN TO THE BAGGER AND CAUGHT FOOT ON THE CORNER OF THE PALLET TWISTING RIGHT KNEE.
Employee was handling material. While attempting to lift material to a platform, some material shifted and began to fall. The employee reached to catch the material injuring their left arm.
Employee was using a dead blow hammer to install pipe in a trench. Employee attempted to swing the hammer to strike a 2x4 piece of wood. The hammer deflected off the side wall of the trench striking the employee in the left hand fracturing their middle finger.
Employee was opening a maintenance trailer door. The hinges on the door failed causing the door to fall. Employee attempted to catch the falling door and suffered a strain in employee's left groin area. The employee waited until 6/12/17 to seek medical attention at which time employee was put on a 25 lb lifting restriction.

Employee was filling and stacking 50lb bags on a wooden pallet. The pallet was only 28 inches high at the time. The employee sustained a rotator cuff sprain.
Employee was stacking 50 lb bags on pallets. employee felt pain in left shoulder as the shift progressed. Employee waited 2 days to seek medical treatment.
Employee was operating a slide gate at main plant screw packer #2. EE improperly grasped the slide while EE was operating it and pinched EE's finger between the hammer portion of the slide and the hammer stop fracturing the end of EE's thumb and causing a hematoma.
Employee was lifting a wooden pallet stacking board off of a set of double stacked pallets. Dirt on top of the board slid when the board was tilted and fell behind the employees' safety glasses. A piece of dirt lodged under EE's eyelid and had to be removed by a Dr.
EE was walking from the employee parking lot to the main property when EE slipped & fell on the ice injuring EE's back. We had previously had over 20 inches of snow over the weekend creating very slippery conditions. Maintenance had cleaned the lot earlier in the morning, but ice under the snow created a slippery condition. The area had been plowed & treated prior to the injury.

Employee was re-inserting the side rails into the slots on the bed of the truck when the rail slipped and lacerated and fractured his middle finger on the left hand during the fall.
IE and another EE were lifting the Rotex screen lid when the lid slipped from the grip of IE creating strain on his lower back and leg.

Employee was stacking 50 # bags of product onto a pallet. Employee stated that he had turned wrong to place the bag on pallet and had a sharp pain in his right shoulder blade that went to his neck and shoulder with temporary loss of breath and face/arms and hands were tingling.
Employee was changing a dust collector sack without turning off the rotary airlock, he had been trained and understood that the airlock must be turned off prior to changing sacks, he had properly performed this task many times previously.
Employee was descending the stairway at the screen plant. It had previously rained and washed material and water to the bottom of the stairway. He did not notice this at the bottom of the stairway and slipped hyper-extending his left knee.
Employee was assisting in changing a screen on a rotex screen frame. As the old screen was being removed from the frame, it unexpectedly rolled up and a frayed wire from the screen slid up behind his safety glasses and caught him in the left eye.

EE had loaded the 928G front end loader onto a lowboy trailer. He got our of the loader and was climbing down the ladder. He missed the last step and fell backwards onto the ground landing on his shoulders and hitting the back of his head on the pavement.
EE was cutting pieces off of a roll of foll and feeding them into the crusher. He tried to reposition the roll and it rolled off of the pallets and landed against his knees pinning him against the wall. Another EE helped him life the roll off of his knees. EE felt something in his back pull when they were lifting the roll.
Employee was getting down from the top of a sack full of material. He was bracing himself on a cement post. His hand slipped off of the post and he fell against the post hitting his ribs on the left side.
Employee was untangling the hose on a Shrink Wrap Gun. While he was untangling it the gun slid off of a pallet of material, swung on the hose and struck him on the right knee.`

Employee was lifting a 50# bag of carbon to dump into the blender feed bin. He lost grip of the bag with his right hand so all of the weight was supported by his left hand. He felt a pull in his left arm. Later that night he continued to have discomfort in his left arm. He went to the doctor for evaluation.
Employee was removing trash from the main plant with a forklift. Some of the trash fell off. He stopped to pick it up. He slipped off the step when he was getting off the Forklift. He caught himself before he hit the floor.
Employee was going down the steps by the Jeffrey Hammermill carrying a broom and shovel. He slipped on a step and started to fall. He grabbed the handrail with his right hand. As he fell his arm was pulled back and upward.
Employee was using a hook bar to reposition a large piece of Graphite that was jammed in the Single Roll Crusher. It did not move on his first attempt. When he pulled harder on the bar he felt a sharp pain in his groin area.

Employee was cutting sheets of plywood into 42" x 48" sheets for stacking boards. He would cut several pieces and then move them 3 at a time and stack them on a pallet a couple of feet away. This project was completed early in the shift. Later in the shift he reported that he had discomfort in his groin area to the shift supervisor.
EE went to step off a catwalk at the dumping bin. His pants caught on the handle of the forklift wheel chock. This caused him to slip off the catwalk twisting his left ankle.
Employee was clearing brush with a weedwacker at the Deibler St. plant on Friday. Apparently some of the brush had poison Oak/Ivy growing in it. He broke out with poison Oak/Ivy over most of his body on the weekend.
EE was tightening a bolt on the end of the rotary dryer. Co-worker was holding the nut in place with a large wrench to assist EE in tightening the bolt. The large wrench slipped off the nut and co-worker lost control of the wrench. It struck EE in the upper lip breaking his tooth.
EE was stacking 50# bags in the Pellet Plant. He got pain in his left elbow. He could not continue to stack bags. After he reported the injury he told another EE that he started having pain in his elbow the previous Friday when he was operating the 101 packer in the plant.
EE was filling sacks of pellets in the pellet plant. He was hanging the empty sacks on sack rack in the pellet plant. he said he got pain in his right shoulder that continued to worsen throughout the shift. It eventually got so bad he could not continue working. EE had reported some pain in shoulder to the shift foreman earlier in the shift and said having pain for a month.

EE was attempting to loosen the drain plug from the gear reducer on the Cedar Rapids Shaker. The wrench slid off the plug causing him to lose his balance and fall against the shaker. His tooth hit against the shaker and was chipped.
EE was plugging a 480v plug into a receptacle and a thin sliver of metal lodged in his right palm. EE removed the metal but the following morning his right hand and forearm were very swollen. He went to the hospital for treatment.
EE was lifting a 50 lb. bag to stack on a pallet. He felt a pop in his right shoulder.
Employee was getting off the forklift below the rotex #2 screw packer where there is a sloped floor. His left ankle turned and strained/sprained his ankle.
Employee noticed pain in his right knee before his break at 6:10 pm. When he went back to work after the break he stooped over to stenciling a sack of material and felt a pop in his right knee. After that he had excruciating pain in the knee.

EE was walking down the hall at a fast pace. She tripped and fell. After the fall I noticed that her right shoelace was untied and dangling. there was also a rug in that part of the hall. It is unclear if she tripped over the rug or the shoelace. The rug appeared to be undisturbed.
EE & co-worker were re-wrapping 3787 material that's stored outside. The sacks had some water & material lying on top. When EE lifted a strap some water w/material in it dumped off top of sack onto his rt thigh. He felt the material burn his leg immediately. He flushed leg w/water immediately. When he removed his pants at end of shift he realized the burn/irritation to his thigh.
He was cutting steel plate with a torch and a spark got behind his cutting glasses and burned his right eyelid.
EE was sampling sacks of material with a thief sampler. When he turned the thief shut. The skin on his left index finger got caught in the pinch point of one of the openings of the thief which resulted in a hunk of skin being cut off.
EE was stacking empty wood pallets by hand. The pallets were wet. A pallet slid off the top of the stack and hit his right lower leg just above the boot. The leg was not treated until 3/2/10 when he noticed black and blue at his ankle. We sent him to the company doctor to get it checked out. He was allowed to return to work with no restrictions.

Employee was walking from the 101 packer area to the restroom. When he stepped down from the elevated area by the bulk elevator he tripped on an empty sample tray that had been left on the floor. He fell and hit his left forearm on the concrete ledge.
EE was preparing to put a stack of 4"x21' steel plates in storage. When EE cut the banding the plates sprung loose and fell into a puddle which caused a splatter and some of the fine material and rust slashed in his face. Later in shift his right eye became irritated.
EE was hauling a sack of graphite behind the pellet plant. It was a windy day. He hit a bump and material bounced off the sack into the air. The air carried the material toward him and struck him in the face. A piece of material got around his safety glasses and into his right eye.
Injured was changing the screens on rotex #1. The screen is located near the top of the building and it was extremly hot that day. His dr had recently adjusted his blood pressure and he had been having some trouble with light headiness and dizziness. This may have contributed to the problem.
EE was dumping sacks of AG711 in the pellet plant. He raised a sack over the dumping bin and the sack tore allowing the load to fall. As it fell, it pushed his right arm under the sack and pinned his arm between the sack and the safety rack.
EE was removing a wad of thread from the sewing machine at the main plant. He pulled the thread trhough the sewing machine and extended his arm to the left. When he did, he reached behind a bag kicker panel as it was returning and his hand was pinched between the panel and the back of the kicker.
Injured was stacking 50# bags on a pallet. When he lifted one of the bags he said he felt pain in his left shoulder and neck. He said he has pain when he turns his head.
Employee was lowering a steel plate from above him. It slipped through his hands and hit something causing it to strike him in the mouth which resulted in 2 chipped teeth.
Employee was going up the steps between the Mill #1 area and the warehouse. He slipped and fell on his right knee.
Employee was stacking 50 lb bags at the turntable in the main warehouse. When he picked up a bag from the turntable he felt a pop in his right elbow area. He continued stacking bags for the rest of the day. The next day he complained of pain in the right elbow and went to the hospital for evaluation.
Employee was changing the grate at the end of the Rotary Dryer. He had removed all of the bolts. The grate normally stays in place when the bolts are removed. This time it came loose and fell striking him in the nose and upper lip.
EE reached down to pick up an empty pallet with his left hand, when he lifted the pallet, he felt pain in his left arm. He said the pallet was heavy because it had ice frozen to it.

Ee was in the maintenance shop fabricating a guard for the blower at the dual bins. When he was welding the pieces together he was not putting his welding hood down fast enough when he started welding.
Ee was walking through a narrow walkway in the pellet plant. The walkway was partially blocked on the right hand side. There is a control box on the left hand side. He struck the control box with his left shoulder and fell to the ground. His left shoulder was badly bruised.

Employee was walking through the locker room to go to the restroom. When he turned the corner he slipped and fell. He hit his head on the bottom of the lockers.
EE was using a utility knife to cut open the bottom of a sack. He was pulling the knife toward himself. When the knife completed cutting the sack, he lost control of it and it stabbed him in the right calf muscle.
Employee was welding a piece of steel into place at the end of the Rotary Dryer. He tried to hit the steel with a hammer to position it properly. When he was swinging the hammer downward it hit a piece of steel causing it to glance off the steel into his index finger on his left hand.
Employee was trying to untie the string on the dumping spout on the bottom of a super sack. While he was pulling on the string his hand slipped off the string and he hit his right elbow on the edge of the metal dumping bin.
EE had opened a 5 gallan bucke of roof coating. He started to pick it up & his foot slipped. When the bucket hit the ground the roof coating splashed out of the bucket & hit EE in the face & head. The pain went under his hard hat & safety glassses.
EE was cleaning the maitenance shop & he stood the 20" elevator sprocket upright to clean around it. He then mvoed a piece of steel & bumped the sprocket causing it to fall and hit side of right leg below knee. It peeled some skin off down to his boot.
Employee had removed the bucket from a skid loader to repair it. He had the bucket on a wooden pallet. He had put a block under the bucket so he could get his fingers under it to re-position it. When he was moving the bucket he thought it was over the block of wood but it was not. His fingers got smashed between the bucket and the pallet resulting in a broken finger.

Employee was stacking 50# bags of pulverized carbon. When he tried to push a corner of a bag in to straighten the bag his thumb got caught on the bag and dislocated his thumb. He told his shift foreman that he is double jointed. This may have contributed to the injury.
EE was emptying a sack of material into a bin. When he reached under the saftey rack to cut the strings one of the lifting loops broke and the sack dropped, this forced EE's head into the edge of the bin. He had a small but deep cut on his neck that required 2 stitches.
Employee was lifting bags off the floor and stacking them onto a pallet. One of the bags slipped and when he moved to try to catch it his foot got caught in the skid and he lost his balance. He put his left hand out to break the fall and his wrist bent back real far.
Employee was reaching over the dumping bin to shake the last of the material out of a sack. The bottom of his rib cage got caught on the lip on the front of the dumping bin. Surface conditions were slippery because of graphite dust in the area.
EE were lifting the lid off of the #1 rotex screen. He felt a pinch in his back.

Employee was operating a weed wacker in front of the lab area. He disturbed a nest of yellow jackets that was in the ground. He couldn't hear them swarm because of the noise of the weed wacker. He was struck many times on the head, neck, arms and leg areas.
EMPLOYEE WAS ATTEMPTING TO CONNECT A PNEUMATIC HOSE TO A PNEUMATIC CONVEYOR. HE WAS TRYING TO ALIGN THE HOSE WITH THE CONNECTOR BY FORCEFULLY BENDING AND PULLING THE HOSE TOWARD HIMSELF. THE HOSE CONNECTOR SLIPPED OUT OF THE SOCKET AND SWUNG OVER AND STRUCK HIM ON HIS SAFETY GLASSES WHICH CUT THE LEFT EYEBROW.
EE WAS STACKING 50-LB. BAGS WHEN HE HAD PAIN IN HIS ARMS.
EE WAS CHANGING A 4" ROLLER ON THE PALLET TURNER AND THE ROLLER BEGAN TO FALL. HE TRIED TO CATCH THE ROLLER WITH HIS LEFT HAND. HE HAD PAIN IN HIS LEFT HAND & ARM WHICH INCREASED OVER THE WEEKEND.

EMPLOYEE WAS REMOVING BAGS ONE AT A TIME FROM A PALLET CONTAINING 50 BAGS THE BAGS WERE GLUED TOGETHER TO HOLD THEM ON THE PALLET EMPLOYEE WAS PUSHING THE BAG TO REMOVE IT WHEN SHE STRAINED H ER MUSCLES IN THE RT PART OF HER CHEST
EMP WAS REACHING TO PICK UP A 50 LB BAG STACKED O A PALLET THE BAGS ARE GLUED TO THE SURROUNDING BAGS EMP PULLED A MUSCLE IN RT FOREARM WHILE PULLING ON THE BAG THIS EMP HAS A TENDENCY TO GE STRAINED MUSCLES OTHER EMPLPYEES DO THIS SAME JOB ACTIVITY WITH NO PROBLEM

EMPLOYEE WAS PULLING DOWN ON BAR TO TIGHTEN BOLT SAID THIS MADE HIS RIBS HURT EMPLOYEE ALSO ATATED THAT HIS RIBS HAD BOTHERED HIM TWO MONTHS AGO IN NONWORK SITUATION
THE TUBE WHEN HIS MIDDLE FINGER OF HIS RIGHT HAND CAME INTO CONTACT WITH THE AIRLOCK FAN BLADE OF THE DUST COLLECTOR. MIDDLE FINGER WAS LACERATED. EMPLOYEE DID NOT ASK SUPERVISOR IF HE COULD C HECK THIS MACHINE NOR DID EMPLOYEE SHUT OOF THE MACHINE.
