Meridian Brick LLC · Struck against a moving object

Operator of Tandem dump truck was driving a loaded truck on the haul road to the dump area. Driver fell asleep and veered off the road contacting the berm on the side. Driver attempted to correct the truck and caused truck to over turn onto the roadway. Driver sustained lacerations to the right hand and left elbow. Driver returned to work 7-03-19.

Mechanic was taking hydraulic pump off a backhoe and it slipped and fell on his foot. Diagnosed at first with contusion of right great toe. Followup visit on 11/3/05 showed fracture. Referred to orthopedic on 11/4/05.

DRIVER WAS OPERATING TRUCK IN HEAVY RAIN. HAUL ROAD WAS VERY SLICK AND TRUCK SLID SLOWLY INTO BERM AT STEEP ANGLE, THEN TURNED OVER. DRIVER CUT LEG WHILE CLIMBING OUT OF TURNED-OVER TRUCK. ALSO SUFFERED LUMBAR STRAIN. TAKEN TO E.R. AT DOCTOR'S HOSPITAL FOR TREATMENT WHERE HE RECEIVED SUTURES.
