After splicing a conveyor belt while attempting to cut a trim section with a utility knife lacerated left thumb requiring 6 stitches.
While tightening and pulling on an eye bolt on a conveyor idler the socket handle slipped striking the employee in the mouth dislodging the front tooth and loosening two others

The IP started a gasoline powered air compressor. Rust blew from the muffler under the IP's safety glasses and into his right eye. The rust had to be removed and a prescription was given.
EE was stepping into a skiff (anchored on shore) to then step onto the dredge. His supervisor told him not to go to the dredge. When EE turned to get off the skiff, he lost his balance,fell and dislocated his shoulder.

The IP needed to retrieve a piece of HDPU from the bellypan under a screen. The IP decided to slide down the side of the bellypan. When the IP slid down the side of the bellypan, his right foot struck the side when he reached the bottom. This caused pain in his right foot. The incident was not reported until 7/30. IP went to the DR on 7/30 and was diagnosed with 2 fractures.

IP states that he was assisting clear material that bridged the inside of the clam shell gate that regulates the flow of material from a hopper onto a conveyor belt. IF lifted an extension pipe with his shoulder to open the gate. At this time the IP felt a small pain where the extension pipe contacted his left shoulder.
IP was cleaning out the cattle guard with the hose from the water truck. IP had turned the PTO off and was walking away from the truck when he slipped and fell. The IP landed on his right shoulder. IP was taken to the clinic for eval and prescribed pain medication. IP has not been placed on light duty yet.
IP was moving sawhorses in the welding shop. As the IP moved one of the sawhorses, a piece of angle iron fell from the sawhorse. The IP's foot was in the line of fire and was struck by the dropped object. The IP sustained a fracture to the little toe on his left foot.
