On 2.20.24, EE was pulling & lifting on an empty battery tray on a CAT 349 Excavator trying to remove it. It did not move, so EE used pry bar to remove. EE reported back soreness the next day but did not want to go to doctor. EE requested to see doctor on 2.24.25 & doctor released to return work. On 2.25.25 doctor required physical therapy and allowed EE to return to work.

PM Technician completed their Pre-Shift Inspection on their truck at the beginning of their shift. The hood did not close properly, so the operator rocked it with their hands in an attempt for it to close properly. When the hood closed, it smashed the operator's right middle finger, causing a laceration on the top and bottom of the operator's finger requiring stitches.

On 5/15/24, EE removed a car jack stand from work area. Put their right hand around the center column, while using their left hand to remove the safety pin holding the center column. When pin was removed, the column dropped, causing right thumb to be smashed by the Saddle. EE did not report injury until 5/28/24. Took EE to have thumb evaluated and found it fractured.

Employee was working with a crew clearing accumulation at the 8x20 primary screen. Employee walked away from primary screen to inspect #3 Drive Motor Belts. While looking up to inspect the belts, a falling rock struck the employee on EE's hard hat / face.

While removing/ attempting to throw a 20 lb rock from the cone crusher, the miner struck right index finger on a lifting eye with a sharp edge. EE was treated at the hospital diagnosed with partial amputation, returned to work same day, operated truck for 8 days until EE saw a hand surgeon 5/12 and was placed off work for 3 days returning 5/16.

After detaching an impact idler assembly, the miner attempted to push the assembly through the center of the conveyor frame for removal. While doing so the miner pinched and lacerated right thumb between the assembly and the conveyor frame requiring stitches.

Contract employee noticed smoke coming from haul unit. EE placed unit in park and exited cab to inspect engine. After noticing a fire in the engine compartment EE jumped over the rail of the 770 haul unit and fractured left leg when landing on the ground.

On 4/17 QC samples were being collected from a conveyor belt. Lockout requirements were not followed. The belt was accidentally activated by another employee for a few seconds. IP's knee contacted a conveyor x-member. No reportable injury was communicated. On 4/28, IP reported that while walking at a lake his knee gave out on 4/26. IP came to work on 4/28 on crutches & light duty.

Technician was installing torque converter which was on blocks, into a 769B off road dump vehicle. He placed straps under converter attempting to center it. When technician tried to push converter into position, strap slipped off to the side. Converter fell on technician's left foot causing fracture.
