HE WAS DISASSEMBLING THE CONVEYOR USING A HAMMER& PUNCH WHEN THE PIN CAME LOOSE. THE CONVEYOR SHIFTED & CAUGHT HIS LEFT INDEX FINGER BETWEEN THEPUNCH & CATWALK.
