EE AND OTHER EE WERE DISMANTLING A 4600 MANITOWOC DRAGLINE. THEY WERE CRANING THE SHEET METAL OF THE HOUSE ON THE DRAG. THE EYE OF THE HOOK BROKE. IT KNOCKED EES HARD HAT OFF AND HIT HIM IN TH E CENTER LOWER FOREHEAD AND ON THE LEFT SIDE OF THE LEFT EYE. THE CENTER LOWER FOREHEAD RECEIVED A LACERATION WHICH REQUIRED STITCHES. I,NAME WAS USING A RAG TO CLEAN AROUND THE BIG ENGINE ON THE 4600 MANITOWOC DRAGLINE WITH THE GUARD IN PLACE. HE GOT HIS RING FINGER ON HIS LEFT HAND CAUGHT BETWEEN THE FAN BELT AND PULLEY ON THE CHARGING PUMP. HE WRAPPED THE RAG AROUND HIS FINGER AND OTHER EMPLOYEE SHUT DOWN THE DRAGLINE AND IMMEDIATELY TOOK HIM TO THE HOSPITAL IN HIS PICKUP. HIS FINGER WAS NEARLY SEVERED AND THE
