Employee was operating a drill and as employee was adding steel and adjusting the centering collar, the drill steel shifted and employee's right hand that was resting on the collar was smashed between the steel and the collar. Resulting in a broken pinky finger tip.

The employee was trying to the close the check valve to repair a hole in a pump hose. It appeared that the valve was broken causing the flap mechanism to be improperly seated and not sealing off the water completely. The employee was trying to realign the check valve flap by opening the valve and closing it again, EE felt a pop in left shoulder.

Loader operator was feeling lightheaded believing BP medication was off. Employee was taken to hospital and after diagnosis it was determined that employee was experiencing a heart attack due to blockage in an artery that required a stent.

Employee was replacing the anvils on a VSI crusher. The employees then used a 20 lb sledge hammer to loosen the anvils from the anvil ring. As the employee struck the 21st anvil, with the sledge hammer, a metal sliver flew out striking the employee on the right lower leg (shin). It was surgically removed.

3 employees were tasked with fusing 18" HDPE pipe to re-route a main water line. The injured employee was positioned at the pipe fuser. While the skidsteer was lowering the pipe onto the stand; the pipe got caught on the bottom clamp. The injured employee attempted to line the 2 pipes up; the pipe shifted causing the employee's hand to get crushed and pinched between the 2 pipes.

Employee was using a chipping hammer when a piece of metal broke loose and hit him in the cheek.

Employee was forcibly attaching - pushing - one end of a new air hose onto a fitting attached to the drill. While pushing, he felt a "pop" in his right elbow, with pain traveling to his right shoulder.

A conveyor belt split in half. During the repair, a wedge was used to lift a hopper. When the wedge was knocked out it struck the employee on the right cheek causing a 1.5 cm laceration.

EE was repairing a headlight bracket on a Cat 992 loader when he slip and hit his right elbow on the bracket

Hernia caused by over exertion or strain

Foreign object entered right eye during a welding operation

EE performs daily equipment repairs. He alleges his back has been bothering him off and on at times (he weight-lifts at a gym every morning) but while at work he lifted a hammer and he felt a low back pain. He notified his supervisor-who then sent him to see a Doctor. It was determined that he has a herniated disc-so he decided he wants it to be surgically repaired.

While employee was working on repairing a conveyor belt he hit himself in the abdomen with a pry bar. He will need surgical repair of his ruptured hernia.

Employee developed a back strain while driving a haul truck.

While performing repair work on a 992G Cat Loader, employee was twisting his body while working with a wrench to tighten a bolt on a transmission and felt pain in his abdominal area.

While repairing a concrete jaw crusher,lifted an object and felt an inguinal groin strain.

EE was performing repair work to his truck, when he caught his foot between two pieces of equipment, he turned and twisted his knee. The injury caused a serve strain to his knee.

EE was loading a truck with equipment and felt a strong pull in his right groin area.

EE was going to take a bathroom break. EE was getting down off of loader, misjudged the step, loss footing. While trying to regain balance EE fell and struck shoulder. Bicep strain.

EE was lifting a drive shaft when he felt a pulling sensation around his belly button area. He was diagnosed with an umbilical hernia, he will have it repaired by his personal general surgeon. per call to Karen Jones 3-21-05, hernia repaired. coder 12

EE WAS ASSESING THE CONDITION OF THE CONVEYER, COMING OUT OF THE TUNNEL. HE TURNED TO GO AND SHUT THE LINE DOWN, AND TO GO GET THE TOOLS HE NEEDED FOR HIS REPAIRS. WHILE TURNING HE GOT TOO CLO SE AND HIS GLOVED HAND WENT UP AND OVER THE BRACKET WHICH CAUGHT HIS FINGERS BETWEEN A GUIDE ROLLER AND THE BELT.

EE IS A LOADER OPERATOR. DURING A BREAK HE WAS OFF HIS LOADER WALKING ALONG THE ROADWAY, HE OBSERVED A BOULDER THAT HE THOUGHT MIGHT CAUSE SOMEONE A PROBLEM AND TRIED TO PUSH IT TO THE SIDE OF THE ROADWAY. WHILE ATTEMPTING TO MOVE THE ROCK HE RUPTURED HIS BICEP.

WHILE PERFORMING WINTER REPAIRS ON EQUIPMENT, HE SLIPPED AND FELL. STONE CHIPS HAD BEEN SPREAD AROUND THE WORK AREA, BUT DUE TO EXCESSIVE MOISTURE IN AREA, GROUND AND BOTTOM OF WORK BOOTS WERE AFFECTED.

WHILE WORKING WITH A CAT MAINTENANCE PERSON TO CHANGE TEETH ON THE BUCKET OF THE LOADER, HE DROPPED A TOOTH OF THE BUCKET ON HIS FOOT. HE WAS WEARING STEEL TOED SAFETY BOOTS AT THE TIME OF THI S INCIDENT.

WHILE WELDING A PIECE OF METAL ONTO A HOPPER, THE METAL SLIPPED AND STRUCK EMPLOYEE'S RIGHT KNEE.

EE WAS UNDER DR.'S TREATMENT FOR SEVERE HYPERTENSION AND ATRIAL FIBRILLATION. HE SOLLAPSED AT WORK WHILE SPEAKING WITH HIS COWORKERS. HE TWAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS PRONOUNCE D DEAD.

HOOKING UP CONCRETE PIPE TO FRONT END LOADER, TAGGED SLING CHAIN BROKE, CAUSING EE TO SLIP AND HIT HIS HEAD.

AS PER DUPAGE COUNTY CORONER'S REPORT, "DECEDENT DIED OF APPARENT NATURAL CAUSES. NO FOUL PLAY IS SUSPECTED AND NO AUTOPSY WAS REQUESTED." CAUSE OF DEATH WAS A HEMORRHAGE TO THE BRAIN STEM.

EE WAS CLEANING A WALKWAY OF FROZEN MATERIAL, STEPPED BACK AND SLIPPED ON FROZEN.

INJURED WAS WALKING ACROSS THE PLANT AREA ON UNEVEN GROUND. THE AREA WAS SLICK AND HE SLIPPED AND FELL. HE LOST NO TIME. THEN ON 4-1-94 A MEDICAL OFFICE INFORMED US THAT HE WAS GOING TO REC EIVE TREATMENT FOR HIS KNEE STRAIN.

JOE WAS WALKING THROUGH PLANT ON SATURDAY WHILE PERFORMING SERVICE WORK. HE ALLEDGES HE TWISTED HIS ANKLE, BUT FAILED TO REPORT THE INCIDENT ONSATURDAY. ON MONDAY HIS WIFE CALLED AND STATED, "JOE TWISTED HIS ANKLE AND HAD SEEN HIS OWN PHYSCIAN." WE REQUESTED THAT JOE SEE A COMPANY DOCTOR, AND WHILE AT THE CLINIC HIS WIFE WOULD NOT ALLOW THE COMPANY DR. TO EXAMINE JOE/NO MED. HIST.

WHILE INJURED WAS CLEANING OFF TOOL BENCH, HE PUSHED AN OLD VISE ON TO HIS FOOT.

EE WAS ASSISTING IN THE ASSEMBLING OF NEW FRAMEWORK. CLEVIS FELL AND HIS FINGER WAS CAUGHT BETWEEN FRAMEWORK AND CLEVIS.

13357EDLY EE TWISTED HIS ANKLE 10 DAYS PRIOR TO REPORTING HE FAILED TO REPORT THE INCIDENT TO HIS SUPERVISOR EE CLAIMS THAT THE ANKLE CONTINUED TO BOTHER HIM SO THEN HE REQUESTED TO BE SEEN BY 01078YSICIAN.

WHILE DRILLING RIG TIPPED OVER.

EE CLAIMS THAT WHILE DRIVING WATER TRUCK HIS BACK WOULD ACHE AT TIMES

WHILE SETTING UP A CONVEYOR EE WAS BENDING & LIFTING & DEVELOPED A BACK STRAIN

EE WAS REPAIRING A PAYHAULER. HE WAS INSTALLING A KING PIN. THE KIN PIN SLIPPED AND STRUCK HIM IN THE HEAD. HE RECEIVED STAPLES TO REPAIR THE LACERATION.

INJUREC WHILE SHOVELING SPILLED MATERIAL, FELT A PAIN IN HIS LOWER BACK. MEDICAL CLINIC FIRST REPORTED AS NOT LOST TIME, INJURED AFTER SEVERAL DAYS, DID NOT FEEL BETTER AND RETURNED TO CLIN IC AND THEN DID NOT RETURN TO WORK.

HE WAS CLEANING A CHUTE WITH A METAL BAR AND CAUGHT HIS FINGER BETWEEN THE BAR AND THE CHUTE.

WAS CLEANING WALKWAY,SLIPPED IN A PIECE OF ROCK.

HE WAS CLIMBING ONTO A DOZER. THE TRUCK WAS WET, HE SLIPPED, THUS CAUSING A STRAIN TO HIS GROIN AREA.

EE WAS ASSISTING THE INSTALLATION OF SOME CONVEYOR SECTIONS. HE WAS STANDING NEXT TO THE CONVEYOR DIRECTING OPERATIONS, WHEN SNOW INSIDE THE BUCKET OF THE LOADER FELL OUT, HIT HIM IN THE BACK, PUSHING HIM AGAINST THE FRAME OF THE CONVEYOR AND KNOCKED HIM DOWN. HE CUT HIS FACE WHEN HE BUMPED UP AGAINST THE FRAME OF THE CONVEYOR.

EE WAS UNLOADING A STEEL PLATE FROM A FORKLIFT TRUCK, PLATE SLIPPED AND CUT HIS FINGER

AS STATED ON A 7000-1, FOR INCIDENT OF 4/26/88, I.NAME HAS HAD ON GOING BACK SYMPTOMS. DUE TO HIS CHRONIC LOW BACK SYNDROME HE NEEDED TO TAKE ADDITIONAL TIME OFF TO REST HIS BACK.

INJURED WAS RPAIRING A CRUSHER.WHILE PICKING UP A CHAIN HE CAUSED A STRAIN TO HIMSELF.HE NEVER REPORTED IT TO HIS SUPERVISOR.HE NOW IS FEELING SOME DISCOMFORT AND REPORTED IT MIGHT HAVE OCCURR ED WHILE REPAIRING THE CRUSHER.TO DATE I HAVE NOT RECEIVED ANY MEDICAL REPORTS,DUE TO THE FACT THAT HE IS SEEING HIS FAMILY PHYSICIAN.

INJURED WAS OPENING A DOOR WHEN APIECE OF STEEL FELL ON HIS LEFT HAND. HE RECEIVED SUTURES FOR THE LACERATIONS AND A SPLINT AS PART OF THE DRESSING. ( THIS WAS REPORTED LATER TO THE COMPANY DOCOTR.)

HE WAS CUTTING AN EXPLOSIVE BAG WITH A KNIFE. HIS HAND SLIPPED CAUSING HIM TO CUT HIS ARM. HE RECEIVED SUTURES AS PART OF THE TREATMENT.

SWEEPING FLOORS WHEN HE HIT LEFT KNEE N BEAM.

EMPLOYEE WAS TRYING TO REPAIR PATCH A HOLE ON A HOPPER BIN A ROCK CAME THROUGH HIT THE PLATE AND CUT HIS THUMB

EMPLOYEE WAS SPLICING A CONVEYOR BELT, BELT SLIPPED, THUS CAUSING EMPLOYEE TO CUT LEG.

BOLTS HOLDING ELECTRIC MOTOR DOWN, CAME LOOSE CAUSING V BELTS TO COME OFF. WHEN CRUSHER OPERATOR AND CONVEYOR MAN INVESTIGATED, THEY ATTEMPTED TO STRAIGHTEN MOTOR, BUT SLIPPED AND FELL OFF PL ATFORM STRIKING CONVEYOR MAN IN CHEST.

I NAME WAS STANDING IN THE SHOP AREA HE BENT OVER TO PICK UP AN EMPTY PAIL UPON STANDING UPRIGHT HIS BACK BEGAN TO HURT.

EMPLOYEE FELT PAIN IN ABDOMEN, AND REQUESTED TO GO HOME. ON 3-26-85 EMPLOYEE STATED THAT HE HAD A HERNIA (LEFT SIDE). AWAITING MEDICAL CONFIRMATION.

EMPLOYEE STATED THAT 3YRS AGO THE SEAT BOTHERED HIM IN THE 180 TRUCK EMPLOYEE CHANGED SEAT ON 2 DIFFERENT TIMES. AFTER THE YEARS ELAPSED HE CLAIMED SEVERAL PAIN IN LEFTT H AND CLAF. EMPLOYEE VISIT THE MD. AND THEN A CHIROPRAOTOR WAS TREATED AND RELEASED EMPLOYEE IS IN SAME VEHICLE AND EXPERIENCING NO PROBLEM SAME SEAT.
