Employee was getting off of the back of water truck. Upon exiting, EE's glove became stuck, lost EE's footing and was swung towards the back bumper which is about chest high. Employee sustained soreness to the ribs. After a CT scan on 05/13/2026, it was discovered the employee had a fractured rib.

Employee was operating a drill and as employee was adding steel and adjusting the centering collar, the drill steel shifted and employee's right hand that was resting on the collar was smashed between the steel and the collar. Resulting in a broken pinky finger tip.

The employee was trying to the close the check valve to repair a hole in a pump hose. It appeared that the valve was broken causing the flap mechanism to be improperly seated and not sealing off the water completely. The employee was trying to realign the check valve flap by opening the valve and closing it again, EE felt a pop in left shoulder.

Loader operator was feeling lightheaded believing BP medication was off. Employee was taken to hospital and after diagnosis it was determined that employee was experiencing a heart attack due to blockage in an artery that required a stent.

Employee was replacing the anvils on a VSI crusher. The employees then used a 20 lb sledge hammer to loosen the anvils from the anvil ring. As the employee struck the 21st anvil, with the sledge hammer, a metal sliver flew out striking the employee on the right lower leg (shin). It was surgically removed.

3 employees were tasked with fusing 18" HDPE pipe to re-route a main water line. The injured employee was positioned at the pipe fuser. While the skidsteer was lowering the pipe onto the stand; the pipe got caught on the bottom clamp. The injured employee attempted to line the 2 pipes up; the pipe shifted causing the employee's hand to get crushed and pinched between the 2 pipes.

He alleges his back has been bothering him off and on at times (he weight-lifts at a gym every morning) but while at work he lifted a hammer and he felt a low back pain. He notified his supervisor-who then sent him to see a Doctor. It was determined that he has a herniated disc-so he decided he wants it to be surgically repaired.

WHILE TURNING HE GOT TOO CLO SE AND HIS GLOVED HAND WENT UP AND OVER THE BRACKET WHICH CAUGHT HIS FINGERS BETWEEN A GUIDE ROLLER AND THE BELT.

HE WAS STANDING NEXT TO THE CONVEYOR DIRECTING OPERATIONS, WHEN SNOW INSIDE THE BUCKET OF THE LOADER FELL OUT, HIT HIM IN THE BACK, PUSHING HIM AGAINST THE FRAME OF THE CONVEYOR AND KNOCKED HIM DOWN. HE CUT HIS FACE WHEN HE BUMPED UP AGAINST THE FRAME OF THE CONVEYOR.
