THE EE INJURED HIS BACK WHILE ATTEMPTING TO LIFT A STEEL DOCK BOARD. THE DOCK BOARD WAS WEDGED BETWEEN AN OLD MECHANICAL DOCK BOARD AND THE CONCRETE FLOOR OF THE WAREHOUSE.

MAINTENANCE CREW WAS USING A HOISTING STRAP AND FORKLIFT TO RAISE THE STEEL TOP OFF OF A JET PULVERIZER MILL WHEN THE STRAP SLIPPED FROM THE FOR5KS. THE STEEL PLATE DROPPED APPROX. 1/2" ONTO T HE MAINTENANCE EE'S MIDDLE FINGER. THE BONE IN THE TIP OF HIS MIDDLE FINGER WAS FRACTURED. HE WAS RELEASED FROM THE DOCTOR AND CLEARED TO RETURN TO WORK.

DR CORT AT SPRUCE PINE HOPSITAL CLEANED WOUND AND PUT STITCHES IN FIRST TWO FINGERS OF RIGHT HAND.
