EMPLOYEE WAS STANDING ON THE BUMPER OF THE TRUCK (PICKUP) THAT TOOK HIM ON THE HILL TO HIS DOZER. AS HE REACHED INTO THE BED OF THE PICKUP TO GET HIS LUNCH BUCKET HIS FEET SLIPPED AND HE FELL AND HIT HIS RIBS AGAINST THE TAILGATE. THE EMPLOYEE DID NOT FELL THAT HE WAS HURT AND DID NOT SEEK MEDICAL ATTENTION UNTIL DEC. 10, 1991.

EMPLOYEE WAS WORKING TO REALIGN THE TABLE BUSHING AND THE DECK OF THE DRILL. HE HAD RAISED THE DECK BUSHING AND LOCKED IT IN PLACE WITH THE DECK WRENCH. HE HAD HIS HAND BETWEEN THE TABLE BUSHI NG AND THE DECK OF THE DRILL WHEN THE WRENCH SLIPPED CAUSING THE BUSHING TO FALL AND FOUL HIS FINGER BETWEEN THE BUSHING AND THE DECK OF THE DRILL.
