THE INJURED EE WAS IN THE PROCESS OF CHANGING OUT THE TOP DECK SCREEN ON THE 5 X 12 TYLER SCREENIN THE CRUSHER HOUSE WHEN HE STRUCK HIS FINGER WITH A HAMMER. INJURY REQUIRED ONE STITCH.
EMPLOYEE ALLEDGES HE WAS INJURED WHEN THE BUCKET CONTROL JACK BROKE ON THE FRONT END LOADER. PHYSICAL EVIDENCE DOES NOT SUPPORT AN INJURY WOULD OCCUR DUE TO THIS CONDITION. INVESTIGATION IS PE NDING MEDICAL DIAGNOSIS AND ADDITIONAL EVIDENCE.

A BLOCK OF COAL WAS JAMMED IN THE RAW COAL FEEDER CHUTE DOOR.THE INJURED JERKED ON THE STEEL CABLE,WHICH OPENS AND CLOSES THE CHUTE DOOR IN ORDER TO DISLODGE THE WEDGED COAL.WHEN HE JERKED ON THE STEL ROPE IT BROKE CAUSING HIM TO FALL BACKWARDS ON HIS BACK.THIS RESULTED IN A BRUISED BACK.THIS RESULTED IN A BRUISED BACK WHEN HE STRUCK THE CONCRETE.
