Employee was walking across the bench area, stepped on a loose rock slipped and fell. Resulting in a hairline fracture of the right fibula

Employee was operating a Mack Coal truck when the truck struck a pot hole. The air ride seat lost air causing the employees seat to bottom out, injuring his lower back. The employee was placed off work due to a diagnosis of bulging disk's.

While cleaning rock off miner and trouble shooting leak. Rock piled up by the miner from what had been cleaned away from pulling out. Member was turned away to leave, when rock slid down from pile against the calf of his right leg and smashed it into another rock.
